Palliative Care Nurse Practitioner
Part- Time Palliative Care Nurse Practitioner – Community- Based for Adult Population, covering the Northern Cook and Lake County, IL:
Job Description:
· Must be a licensed advanced practice nurse with palliative care training or education.
· Possesses the ability to assess, diagnose, and treat patients with life limiting illnesses.
· Must be knowledgeable and skilled in managing a wide range of chronic and acute medical conditions.
· Provides emotional support to patients and family members while incorporating holistic approach and care coordination.
· Develops a treatment plan to address pain, nausea, breathlessness, anxiety and other presenting symptoms.
· Able to work independently and order medications, medical services, or diagnostic tests as appropriate.
· Able to travel and see patients in the home, independent or assisted living facilities, nursing homes, and hospitals.
