For over two decades, Apex Home Care Services has supported individuals and families in Western Massachusetts with quality in-home care. Under the supervision of a Registered Nurse, our Certified Nursing Assistants and Home Care Aides provide personal care and homemaking services to clients in Greater Springfield and Western Massachusetts, including Hampden County, Hampshire County and many towns in the Pioneer Valley.

Job Description – Certified Personal Care Homemaker

Qualifications:

1. 18 years of age or older

2. Reliable means of transportation

3. Statement of good health form MD

4. Up to date Mantoux/TB testing

5. Acceptable Cori Screening

6. Verbal and written communication skills consistent with job requirements

7. Emotional maturity to function independently in the home

8. Sensitivity to the physical, social and emotional needs of patients in the home setting

9. Certificate from Approved Personal Care training program

10. Skills Checklist signed by Registered Nurse

11. Test scores, which reflect competency in training

Reports to:

1. Coordinator

2. RN Field Supervisor

Responsibilities:

1. Routine light housekeeping such as vacuuming, light laundry, dusting, dry mopping, dish washing, changing bed linens, kitchen and bathroom cleaning, emptying trash.

2. Shopping (1x weekly, nearest grocery store, pharmacy)

3. Prepares and serves nutritious meals according to prescribed diets

4. Follow all activities on the personal care plan provided by the RN Supervisor.

5. Assist with task of personal hygiene, which may include sponge or tub bath, hair grooming, or shampooing, foot care (excluding nail cutting) and assistance with dressing.

6. Cleaning of personal appliances such as eyeglasses and dentures

7. Shaving (limited to electric razors only)

8. Assisting with toileting.

9. Assisting with ambulating.

10. Assisting with transferring (excluding transfer of totally dependent clients)

11. Provides a safe and comfortable environment for the clients.

12. Reports all changes in client’s mental or physical condition or in the home situation to Coordinator or Field Supervisor.

13. Adheres to Polices and Procedures, and job description.

14. Attend in-service to maintain valid certification.

15. Communicate client change(s) to coordinator.

16. Remind the client to take prescribed medication(s) as reviewed and approved by RN.
