JOB SUMMARY

• Provides professional occupational therapy diagnostic and therapeutic services by collaborating with physician and interdisciplinary team members to develop and manage the patient's plan of care.
• Provides ongoing evaluation of patients status and goals, modifying treatment program for timely progression.
• Observe/Assess the progress of the patient & prepare detailed progress reports.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers
• Develops comprehensive goals for patient treatment to maximize patient outcomes.
• Instructs and advises clients, family and other health team personnel including, when appropriate, home health aides, in the phases of occupational therapy in which they may assist the patient.
• Provides documentation according to policy.
• Ensure compliance with relevant laws, regulations, or standards.
• Other duties as assigned.

EDUCATION

• Bachelors degree required; Masters degree preferred.
• Graduate of an accredited Occupational Therapy Program.

EXPERIENCE

• Minimum of one year of experience post-license in occupational therapy.
• Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and other adult population.
• Demonstrated knowledge of the principles of growth and development over the life span.
• Able to assess data reflecting the patients status and interpret the appropriate information needed to identify each patients requirements relative to their age-specific needs.

CERTIFICATIONS/LICENSURE

• Valid New York State license as an Occupational Therapist.

KNOWLEDGE AND SKILLS

• Excellent communication skills and the ability to interact well with diverse individuals.
• Experience with territory management, strong presentation skills, performance management, building relationships, emphasizing excellence, negotiation, results driven, sales planning and execution.
