Occupational Therapist Job at Visiting Nurse and Hospice for VT and NH

Screen, evaluate and re-evaluate patients referred by a licensed physician. Assess patient's functional ability pre- and post-operatively, pre- and post-treatment, upon admission and discharge.

Provide direct treatment to patients for maximal progress in therapeutic process. Treatment goals and objectives must be approved by physician and be acceptable to therapist, other health care team members and the patient.

Select therapeutic modalities to meet patient's treatment goals and objectives.

Communicate verbally and in writing to all members of the health care team as needed. Participate in planning for the patient's discharge.

Document evaluations, treatment goals and objectives and patient progress according to agency policy.

Educate health professionals/student in Occupational Therapy skills and responsibilities as appropriate.

Teach patients and family members the rationale for treatment and actual treatment procedures. Provide a mechanism for patient/family follow-up.

Participate in the agency's continuous quality improvement program.

Participate in public relations programs designed to teach the basis of Occupational Therapy.

Keep abreast of new developments in Occupational Therapy treatment and services.

Keep knowledgeable regarding homecare regulations.

Must be licensed in Occupational Therapy in Vermont and New Hampshire as appropriate.

BS in Occupational Therapy or post graduate Occupational Therapy Certification program. Passage of National Registration Exam.

PURPOSE
At the direction of the Clinical Manager, screen, evaluate, re-evaluate and treat patients referred by a licensed physician. Coordinate and communicate treatment activities with health care team members and the patient. Performance evaluations will be the responsibility of the Clinical Manager.
