Position Summary:
Under the direct supervision of the Access to Care Supervisor, the Access to Care specialist is responsible for monitoring all activities related to companionship arrangements, for making confirmation calls and last-minute cancelations.

Essential Functions of the Job:
  • Responsible for ensuring all companionship needs related to transportation for participants are address appropriately.
  • Provide guidance, for all staff attending companionship.
  • Main point of contact for inquiries from staff while out on companionship.
  • Create companionship list for PACE locations.
  • Perform reminder calls to participants to confirm all specialty appointments including the need for companionship 48 hours prior to appointment date and record outcome on Participant’s health record.
  • Provide monthly report to management of reminder call outcome.
  • Perform reminder calls to Specialist offices to confirm all appointments including the ones that require companionship 48 hours prior to appointment date and record outcome on Participant’s health record.
  • Send confirmed companionship list 24 hours in advance to floor coordinator for CNA/PCA assignment.
  • In Collaboration with Floor Coordinator to develop an accurate companionship list on a daily basis.
  • Works closely with other departments to ensure efficiency in processes.
  • Prepares reports requested by supervisor where information must be obtained from a variety of sources.
Additional Duties and Responsibilities:
  • Communicates effectively with participants and co-workers.
  • Works effectively in a team setting.
  • Adheres to SYHC and PACE attendance and punctuality policies and practices.
  • Maintains all records and documentation as required.
  • Performs other duties as assigned.
Job Requirements: Education (Minimum level of education):
  • HS Diploma/GED Equivalent is required.
  • Post High School Vocational or Specialized Training preferred
Experience (Minimum level of experience):
At least 6 months in a customer service or administrative role.

Verbal and Written Skills Required to Perform the Job:
Bilingual (English/Spanish) required.

Equipment Used:
Phone, computer, Microsoft Office programs (especially Excel).

Working Conditions and Physical Requirements:
Office environment, prolonged sitting, standing, walking.
