Part-time Advanced Practice Provider – Thornton

The Practice: A small primary care office looking for an Advanced Practice Provider to join an established clinic. This position offers extensive opportunity for collaboration and flexibility to customize the role.

Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Community: Located in Thornton, CO the practice is centrally located between Denver and Boulder and services a diverse patient population.

The Position:

• Provides full spectrum primary care.
• Develops and documents a plan of care for each patient, including a complete medical history, a physical examination, diagnosis of the causes of injuries and illnesses, appropriate treatment and/or referral, and hospital visitations when necessary.
• Adheres to departmental policies, procedures and objectives, ongoing quality improvement objectives, and safety, environmental and infection control standards.
• Maintains patient confidentiality and complies with all federal and state health information privacy laws.
• Records complete, timely and legible medical records.
• Open to seeing patients of all ages and providing women's health services within a primary care setting.
• Comfortable seeing patients with chronic and complicated health conditions.

Qualifications (Education/Experience/Knowledge/Skills/Abilities):

• 3+ years of experience in primary care preferred.
• Physician Assistant or Nurse Practitioner credentials from an accredited institution.
• Excellent communication and inter-personal skills
• Medicare/Medicaid credentialing a plus.

Salary Range: $47-$55 per hour, dependent on experience
