Our Medicare ACO is one of the highest performing ACOs in NYS. We achieve this through team-based primary care. We are seeking patient advocates to work with our RN based team to ensure that patients receive the care they need, including: prevention and screening, understanding of their diseases and how to manage; and active engagement in their health to help ensure they live their very best life. We partner with our caring primary care doctors and nurses to create a safety net in-between doctors visits.
- Do you enjoy talking to Medicare patients about their needs and concerns?
- Are you comfortable on the telephone speaking with patients?
- Do you want to learn more about chronic conditions?
- Can you quickly establish rapport and trust with patients over the telephone?
- Do you want to help patients get all the preventive care and services they are entitled to under Medicare?
- Do you have high energy and telephone skills to make sure patients know they have caring support to help them?
- Are you highly organized to help nurses and doctors organize their records and paperwork?
- Do you like being an "investigator" and calling home health companies, equipment vendors ad pharmacies to make sure patients get the services that their doctors ordered?
- Are you a problem-solver?
If you answered yes to these questions, you are a candidate for the Patient Advocate/Navigator position! The Patient Advocate/Navigator will help the doctors and nurses improve the health of seniors through telephone health surveys/interviews, following up to ensure patients receive preventive care, answering questions, receiving/sending correspondence, following up with nurses for next level of intervention, and being part of a team that helps to guide the patients and their families through the very complex healthcare system. Pay: $20.00 - $24.00 per hour
