We are seeking Clinical Professionals (RN, BSN, LPNs, ADNs, social workers, etc.) to work as Patient Care Coordinators with Chronic Care Patients in a Call Center environment

Fluent Full Bilingual (required).

Description

A Patient Care Coordinator is a health professional that helps to manage a patient's care, for example, the elderly or disabled. They monitor and coordinate patients' treatment plans, educate them about their condition, connect them with health care providers, and evaluate their progress.

In a call center environment, the PCC will service patients by working on their care plan and phone summary provided by the caller. They will help build a detailed care plan for patients to be viewed by the physician and fill in all the gaps of the care plan so that the physician can re-calibrate medication dosages, address patients’ needs, and refer patients to specialists when needed.

Responsibilities:

• Manage patient care
• Patient screening
• Connect with new patients via phone and email
• Process pharmacy orders and respond to inquiries and issues regarding said orders
• Review medication inventory
• Arrange and coordinate provision of special equipment or therapy for at-home care
• Patient case management tasks
• Locate and provide community resources to patients and their family members
• Assist the care team with goal setting

Qualifications:

• Be able to commute to Hato Rey, and work in our facilities, onside.
• Familiarity with medical software and equipment, specially EMR's
• Responsible, punctual, and committed to work.

Experience and Education Required

• Associate’s Degree in healthcare administration or related area of study required, or Bachelor’s Degree preferred (RN, BSN, LPNs, ADNs, or social workers with medical experience)
• Strong communication skills
• Above average critical thinking skills
• One year or more working in a healthcare environment
• Experience with basic computer programs, such as MS Office
• Candidates with knowledge of / familiarity with outpatient therapy services, specialty pharmacies and medical supply companies desired
• Registered Nurses meet educational qualifications

• Full Bilingual (Fluent speaks English, required)

on NY Saving lights period will be from; 10AM to 7PM
