Hackley Community Care (HCC) is a community health center located near the beautiful shores of Lake Michigan and Muskegon Lake. HCC provides medical, dental, behavioral health, and pharmacy services to the residents of Muskegon County and surrounding areas and we are proud to offer a transparent work environment where people choose to work, stay, and play.
Under the direct supervision of the Patient Access Supervisor/Manager, the role of Patient Coordinator is to assist new patients with access and to provide all patients with information regarding resources available at Hackley Community Care Center as well as outside agencies.

Qualifications and Skill Requirements:
- Possess a high school diploma or equivalent
- Ability to work in a team environment or independently
- Ability to maintain effective working relationships
- Ability to maintain confidentiality
- Ability to interact effectively and respectfully with diverse groups
- Efficient typing and Microsoft Office skills
- Possess excellent oral and written communications skills
- Excellent customer services skills
