Position Focus:

We are seeking a Patient Navigator (PN) who will support a research program that works at the intersection of infectious diseases and substance use within the Yale School of Medicine Section of Infectious Disease, AIDS Program. They will be primarily responsible for working with persons who are living with or at risk for HIV along with other infectious diseases who have opioid use and stimulant disorders, and have experience with justice involvement. The PN will help research participants to access treatment for substance use disorders and HIV infection treatment and prevention and will assist them in addressing other identified issues/needs.

The PN will perform the majority of their responsibilities in Tolland, Windham, and Middlesex counties in CT. The position will require travel to these areas.

Assesses research participant needs and assist with linkages to care and treatment for HIV, PrEP and medication for OUD and other care as needed. Engage in conversations with people about their drug use, overdose prevention, HIV and HCV risk. Explores treatment options with participants, with a focus on expanding awareness of and connection to medication treatment programs. Facilitates timely referrals and placements with treatment/recovery support providers Connects participants to the larger community of care. Have knowledge of of community and clinical resources to make active referrals and provide advocacy in obtaining services. Collaborates closely with the patient’s healthcare team, including appropriate community resources, to assure that the patient’s healthcare and other associated needs are being met. Assists patients with making follow-up appointments, filling prescriptions, and other healthcare needs.; Helps patients complete applications for services and benefits to obtain additional assistance. Enters data related to participant encounters into research database. Carries out research within the scope of the established study protocol. Adheres to all human subjects regulations as defined by Yale University and the federal government that pertain to research studies. Performs other duties as assigned within their scope of practice.

Essential Duties

1. Assumes primary responsibility for a program, service, activity, operation or function in an assigned area. Develops objectives, practice and procedure and identifies resources to ensure accomplishment of operational goals. 2. Coordinates day-to-day operations in order to achieve designated goals. Ensures effective management and leads the development and implementation of best practices. 3. Designs and implements quality control metrics to identify areas of risk. Analyzes, recommends, and implements strategic solutions to minimize risk. 4. Develops a variety of complex report and other written materials. 5. Provides guidance to and collaboration with internal and external partners as necessary. 6. Develops, oversees and monitors budgets. 7. Develops and maintains specialized training. 8. Performs other duties as assigned.

Required Education and Experience

Bachelor’s Degree in related field and three years of related experience or an equivalent combination of education and experience.

Required Skill/Ability 1:

Mature, reliable, and able to produce accurate work, possesses the ability to work independently, effectively and efficiently under pressure, and to multi-task, juggling competing demands with limited supervision.

Required Skill/Ability 2:

Excellent communication and interpersonal skills for dealing with internal and external contacts. Ability to develop trust and rapport with research participants, project staff and community partners. Understand and utilize the principles of Harm Reduction. Has skills in active listening and conflict management and experience with culturally diverse populations.

Required Skill/Ability 3:

Highly organized, detail-oriented with the ability to function under deadline pressure. Highly attentive to follow through on logistical, scheduling, and communications with both Yale staff and collaborators.

Required Skill/Ability 4:

Ability to maintain integrity in working with confidential information.

Required Skill/Ability 5:

Computer proficiency including working familiarity with word processing using programs such as Microsoft Word, Excel, and REDCap. Comfort entering data electronically. Ability and desire to learn new computer programs.

Preferred Education, Experience and Skills:

Bachelor’s or Master’s Degree in a related discipline (e.g., Public Health, Epidemiology, Health Education). Preferred Experience: Knowledge and familiarity with HIV, PrEP, and substance use and related treatments and human subject research regulations.

Preferred Licenses or Certifications:

Valid driver’s license. Must have own vehicle to conduct patient navigation responsibilities in Northern and Eastern CT.
