Integrated Health Care is a collaborative, multi-disciplinary team of primary care and mental health professionals, including social workers, marriage and family therapists, licensed mental health counselors, psychiatrists/nurse practitioners and registered nurses. Position Summary:
The Patient Support Representative coordinates the scheduling of Health Monitoring Appointments, as requested by RN or referral, and acts as a liaison between Prescriber Team and other care team members regarding patient needs and outcomes.

Job Responsibilities:
  • Responsible for centralized scheduling for MD and other prescribers, and managing scheduling changes
  • Answer phone lines and respond to general inquires and faxing review, sending and loading into EHR system
  • Communicate significant clinical information to other health care professionals as necessary, send records as necessary to coordinate care
  • Prepare patient record information for physician referrals
  • Communicate in a tactful and professional manner test results to patients and schedule follow-up visits if indicated
  • Prepare patient record information for physician referrals
  • Toxicology Testing prepping and administration
  • Notify clinic administration of staff absence per attendance policy and rotate coverage (including Liberty Resources reception) when needed
  • Attend and actively participate in Clinical Staff meetings and in-services
  • Participate in cultural diversity training and team meetings
  • Maintain patient confidentiality
  • Perform other duties as assigned

Qualifications:
  • One year college/technical school or High School diploma (equivalent) with one year relevant experience.
  • Strong organizational skills, ability to manage competing priorities, and ability to use good judgement in problem solving.
  • Knowledge of medical procedures and medical terminology, and ability to effectively use an electronic medical record.
  • Ability to work collaboratively with a multidisciplinary team
  • Excellent oral and written communication skills
  • Ability to work in a fast-paced outpatient family and behavioral health clinic environment
