ABOUT VALLEY VISTA
Valley Vista is a Permanent Supportive Housing (PSH) development in San Diego that provides homes and on-site supportive services for people who have experienced homelessness. The PSH setting centralizes services on-site to ensure residents have immediate and easy access to the resources necessary for long-term housing stabilization, improving their quality of life and community integration.

WHAT IS A PEER SUPPORT SPECIALIST?
As part of the multidisciplinary PSH team, the Peer Support Specialist builds positive relationships with residents to provide them with assistance and support with their transition to housing, accessing necessary services, and linkage to mainstream resources. Responsibilities of the Peer Support Specialist include:

  • Collaborating with the PSH team and residents to develop individualized service plans (ISP) that are client centered and identify clear objectives
  • Promoting and following the PSH program's established values, guidelines and rules
  • Utilizing motivational interviewing to help residents increase their opportunities and linkages to community resources, decrease social isolation, and address behavioral health and substance use needs
  • Providing case management support including monitoring ISP progress, assisting residents with documentation necessary to remaining in stable housing, and crisis intervention
  • Maintaining documentation standards as set forth by PATH polices and the program contact

WHAT YOU BRING

  • Demonstrated ability to work with diverse communities.
  • Demonstrated ability to maintain and execute confidential information according to HIPAA standards.
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgement.
  • Demonstrated ability to use computers and proficiency in Microsoft Office software particularly.

PREFERRED QUALIFICATIONS

  • Lived experience with homelessness, mental health, or substance use challenges.
  • Peer Support Certification.

MINIMUM QUALIFICATIONS

  • Experience providing direct services to vulnerable individuals.

MANDATORY REQUIREMENTS

  • Employment Eligibility Verification

  • Updated Tuberculosis Test
  • Successful completion of background screening & drug test.
  • Ability to work flexible hours which may include evenings or weekends.
  • Ability to obtain CPR/First Aid certification through PATH.

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.

FLEXIBLE WORK
We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
