Home Health Physical Therapist
$1,000 Sign On Bonus after Completion of Contract
13-week contract
Monday- Friday 8am- 5pm
$ 2,200- $2,600 a week!

Responsibilities for Home Health Physical Therapist:
Processes information gathered during the evaluation to establish an appropriate treatment plan
Performs ongoing re-evaluation of patient's response to treatment, determining appropriate modifications and timely discharge
Ability to teach patient and family using appropriate techniques and documents in record
Provides physical therapy services to patients
Provide home health physical therapy within the Physical Therapy Professional Scope of Practice, to restore function and prevent disability following disease or injury
Assist the patient in reaching maximum ability within the limits of his/her capabilities and the disease process
Assist the physician in evaluating level of function and in developing the plan of care
Evaluate the home environment and make appropriate recommendations
Provides physical therapy services to patients according to a written physician's plan of care Reviews Physician's referral, patient's condition and medical records to determine physical therapy treatment required
Qualifications for home health physical therapist Two (2) years of therapy experience One (1) year of therapy experience in home care or related rehabilitation setting Current license to practice Physical Therapy in the State of Missouri Current Basic Life Support (BLS) for Provider Certification or ability to obtain in 60 days (Renewal of BLS card every 2 years)
