HSS Rehabilitation provided by Stamford Health is seeking a motivated physical therapist to join our Outpatient Rehab team. Stamford Health and Hospital for Special Surgery (HSS) joined to form a groundbreaking collaboration and we are actively seeking candidates to broaden our team of clinical experts. Our current locations include clinics in Stamford, Greenwich, New Canaan, Wilton, and Westport and there are opportunities throughout the region. We treat a wide range of diagnoses and qualified candidates should have experience in non-operative and post-operative patient management. MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES:
Evaluation - Obtains pertinent medical and social history. Performs thorough and accurate evaluations using techniques to objectively measure physical and functional status. Determines functional and measurable short and long term goals and designs an appropriate treatment program. Considers discharge options at time of initial evaluation.
Treatment – Performs treatment as outlined in the initial evaluation. Modifies treatment as necessary, updates goals and re-evaluates discharge plans throughout the course of treatment. Demonstrates thorough knowledge of treatment techniques and applies appropriately. Treatment should include patient and family education and communication/interaction with appropriate team members.
Documentation – Clinical documentation is timely, objective and complete. Administrative documentation including but not limited to billing and outcome measures is accurate and timely.
Education – Assumes personal responsibility for continuing education and professional development. Provides education to therapy staff, system employees and community.
Supervision – Supervises the patient care activities of assistants, aides and volunteers. Delegates routine care to supportive personnel as appropriate. May supervise students.Required SkillsRequired Experience
- Must be licensed by the Connecticut Department of Public Health.
- New graduates considered.
- BLS certification required.
- Graduate of an accredited curriculum with a minimum of a Doctorate of Physical Therapy. For individuals who completed appropriate Physical Therapy education prior to 2015, with a valid Physical Therapy license in the state of practice or proof of license eligibility, a Bachelor’s degree or greater in Physical Therapy will be accepted in lieu of the Doctorate of Physical Therapy degree.
