The Physical Therapy (PT) Case Manager provides and oversees home health services for clients within the community. Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans taking into consideration quality measures and financial aspects of treatment. Provides skilled care to clients and clinical oversight to professional and unlicensed personnel. Builds and maintains respectful relationships with others. The position reports to the Clinical Supervisor.


SCHEDULE

  • Full-time, Monday – Friday between 8 am – 5 pm; flexible schedule
  • 1:1 client care visits are an average of 1 – 3 hours per home
  • You may visit multiple homes in one day

DUTIES AND RESPONSIBILITIES

Client Care

  • Performs initial and periodic assessment of clients to assist physician in determining needs and level of function. Participates in the development of the Plan of Care. Implements and evaluates the effectiveness of care.
    • Uses interview and physical assessment skills to evaluate client needs.
    • Demonstrates therapy skills appropriate for services provided.
    • Records therapy diagnoses, measurable outcomes, interventions and therapy notes according to the company’s standards.
    • Develops and updates care plans as client’s condition, regulations indicate or at least every 60 days.
    • Documents progress reports, updated medical orders and plan of treatment to physician at least every 30 days or more often as per regulatory requirements or changes occur (i.e. 485’s, reassessments).
    • Documents client discharge summary for submission to attending physician.
    • Develops transfer summary for discharged clients referred to other agencies.
  • Provides physical therapy treatments, procedures, evaluations and diagnostic tests for clients under the direction of their physician for whom therapy has been medically prescribed.
    • Translates all exercises into functional activities or activities of daily living.
    • Provides instructions in the use and care of assistive devices.
    • Develops needed plans for modifying equipment, appliances and the physical surroundings in client's homes.
  • Demonstrates problem solving abilities in adapting to the client’s changing needs.
  • Organizes caseload to provide quality care in the most cost-effective manner.
  • Assumes responsibility for coordination of client records in the home and the office.
  • Coordinates client care with family, other caregivers, community resources and other disciplines. Participates in case conferences as indicated.
  • Fosters continuity of care by identifying problems and participating in solutions to reach identified client outcome.
  • Maintains confidentiality of all information pertaining to clients, families and employees.
  • Understands and follows infection control policies.

Supervision and Leadership

  • Oversees Physical Therapy Assistants (PTA) competency performance as observed in the client’s home during supervisory visits. Teaches delegated tasks performed to improve client care.
  • Provides orientation of employees according to company policy.
  • Participates in the performance review of direct care givers.
  • Keeps up to date on state and federal regulations as they apply to home care.
  • Maintains positive and effective communication with employees. Understands and participates in team concept.
  • Demonstrates self-direction to prioritize and accomplish job responsibilities.

Education and Teaching

  • Assesses readiness and ability of the client and significant others to learn and carries out appropriate individualized teaching based on assessment.
  • Provides clients and families with information, support and encouragement to help motivate clients in their progress. Instructs client's family on a home physical therapy program.
    • Assists individuals and families to accept and adjust positively to physical, mental and social limitations.
    • Participates in the development of community resources to meet the needs of clients.
  • Consistently documents teaching and individual and family response of client care.
  • Provides in-service education programs for company employees as requested.

Financial

  • Ensures that all client’s authorization for services are current and the quantity and type of services provided meet payer requirements.
    • Applies basic knowledge of financial resources for care of clients.
    • Fills out all admission and financial forms completely and accurately and distributes them appropriately.
    • Assumes responsibility for regular, continuous assessment and management of client cases in an efficient manner.

Community Relations

  • Develops and maintains a good working relationship with referral sources including physicians, hospitals, nursing homes, community resources and payers.

Lead Duties and Responsibilities

  • Serves as a resource to branch and contracted therapy personnel regarding clinical matters.
    • Educates branch management regarding therapy practices.
    • Ensures compliance with federal and state and company rules and regulations.
    • Assists branch offices with client and employee concerns relating to therapy matters.
    • Assists with the development of branch and contract therapy personnel.
    • Assists with audits of therapy clinical records to maintain compliance.
  • Serves as a resource to the branch office regarding recruitment and retention of therapy personnel.
    • Assists in development of appropriate ads, attendance at trade fairs, etc.
    • Assists branch office in the recruitment and hiring of therapy personnel.
  • Assists General Manager in the continuous improvement of company therapy services and processes
    • Maintain company image as a high quality care health care provider.
    • Maintain and enhance professional and managerial expertise through appropriate educational and organizational activities.


PHYSICAL/ENVIRONMENTAL DEMANDS


  • Heavy to Very Heavy. Involves lifting clients, bending, stooping, stretching, and setting up equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.
  • Exerting in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, and in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for heavy work.

QUALIFICATIONS

  • Graduate of an accredited Physical Therapy program; currently licensed as a Physical Therapist in the state of Minnesota. The license must not have been revoked, suspended, and without limitations or restrictions.
  • Have at least two (2) years experience in a health care setting or equivalent experience. Home care experience preferred.
  • Strong interpersonal, written and oral communication and teaching skills.
  • Strong organizational skills and the ability to work independently.
  • Demonstrates ability to make appropriate clinical judgements.
  • Basic computer skills and ability to learn new software. Knowledge of Word and Excel a plus.
  • Ability to manage a variety of assignments while establishing priorities and assuring that necessary duties are completed.
  • Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.
  • A valid driver’s license and proof of car insurance when using a personal vehicle for company business.
  • Proof of negative mantoux or documentation of negative chest x-ray.
  • Current CPR certification recommended.
  • Have U.S. Citizenship or evidence of valid Alien Work Permit.
  • Discloses any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will not be hired.
  • Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
  • Provide proof you are fully vaccinated against COVID-19. Medical and religious vaccine exemption requests must be approved by the company before your start date.
