Summary:

The Primary Therapist will be responsible for providing mental health and substance abuse treatment services to patients in an inpatient and/or outpatient facility setting. He or she will provide individual, family and group therapy, collaborate with the clinical team in the formulation of proper documentation and assist in coordinating discharge planning.

Primary Therapist Requirements:

  • Graduate from an accredited college or university with a minimum of master’s degree in Psychology, Social Work, Counseling and/or health-related field.
  • LPCC, LMFT, LCSW, CADC II, or other mental health or substance abuse license or certification. MUST.
  • Need to have a minimum of a Master’s degree education.

Primary Therapist Responsibilities:

  • Complete initial meeting to assess all patient/clients admitted to the program.
  • Complete biopsychosocial assessment on all admissions within the organization prescribed time. Biopsychosocial contains all required elements.
  • Complete all clinical documentation on time and with all required elements.
  • Facilitate and/or conduct individual, group and family therapy activities.
  • Assess discharge planning needs upon admission, throughout treatment, and during discharge planning process.
  • Document in the medical record any counseling, education, services, individual and group sessions, family interactions, assessments and reassessments, discharge planning, and any other intervention within the prescribed by the organization.
  • Complete ASAM assessment upon admission, change in level of care, and on discharge. Utilize ASAM assessment to determine appropriate placement.
  • Complete suicide risk assessment as needed utilizing evidence bases, standardized tools.
  • Complete client evaluations utilizing ASI, DSM-V or other diagnostic, standardized, evidence-based tools.
  • Develop initial and comprehensive treatment plan based on interdisciplinary assessments and other diagnostic findings.
  • Conduct interdisciplinary or other treatment team meetings on a regular basis to monitor client progress, achievement of treatment planning goals, treatment issues, discharge planning needs, and other issues.
  • Provide case consultation as needed.
  • Involve family in treatment as needed. Support family program as needed.
  • Participate in crisis intervention actions as needed.
  • Support and help develop the patient/client treatment schedule. Monitor patient/client staff with assigned treatment schedule.
  • Prepare and document discharge summary.
  • Obtain records or prior treatment episodes as needed.
  • Assist in patients/clients in considering and/or accessing programs such as Family Medical Leave, disability, and other services that may support community reintegration, discharge planning, and return to work.
  • Assist in referrals to 12 step and other community programs.
  • Other duties as required to address operational needs.

Primary Therapist Shift:

  • Must be flexible with work schedule.
