Job Title/Position: Registered Dietitian

Reports To: Director of Clinical Services

Job Description Summary:

Plans, organizes, and conducts dietetic education for organization personnel. Functions as a member of the home care team with expertise in nutritional counseling.

Essential Job Functions/Responsibilities:

1. Develops and implements a nutritional plan based on an assessment of nutritional needs. This plan is included in the patient’s plan of care.

2. Evaluates and reports results of nutrition appropriately.

3. Instructs home care personnel to:

  • Provide appropriate counseling to individuals and families/caregivers in the home setting.
  • Evaluate and instruct on nutritional principles, dietary plans, food selection and economics.
  • Adapt plans to the individual's life style, as indicated.
  • Reevaluate nutritional care by providing home follow-up care.

4. Communicates appropriate dietary history and nutritional care data in the clinical record.

5. Records subjective and objective information, assessment, and treatment plan, in the clinical notes including a discharge summary.

6. Communicates with clinician regarding patient’s progress and treatment goals.

7. Maintains confidentiality and safety of patient clinical records.

8. Complies with the Organization's policies and procedures.

9. Performs other duties as assigned.

Position Qualifications:

1. Preferably, holds a Master's degree from an accredited institution with major in foods, nutrition, or dietetics, and successful completion of an internship or equivalent experience.

2. Successfully completed courses in methods and principles of nutrition counseling.

3. A member of and is registered in the American Dietetic Association.

4. Is licensed by appropriate state(s), if applicable.

5. Possesses and maintains current CPR Certification.

6. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
