We provide 12 Memory Care and 20 Assisted Living apartments in a home like setting, fostering strong interpersonal staff and resident relationships!! We love to do activities and have a full time Activities Director, Housing Director and Cook!

We are seeking a Registered Nurse that will ensure the highest level of health and safety for our amazing residents!

No caregiver scheduling/hiring/interviewing, that is all handled by the Housing Director.

Nurse Responsibilities:

  • Serve as the primary Nurse on-site, overseeing the care of all Residents
  • Complete initial, face-to-face comprehensive assessments of prospective/new clients and develop a proposed service plan
  • Monitor and reassess clients' needs and care plans
  • Assures that residents medication records are up to date, that medication administration procedures are followed properly, investigates any errors and issues with staff, and implements corrective action plans as needed
  • Facilitates coordination of client's services, such as hospice, occupational therapy
  • Implement and monitor medications and narcotics for proper set up, administration, storage and disposal procedures
  • Train and supervise the LPN/Caregivers on nursing/medication services for residents
  • Monitor caregivers performance and ensure they are following procedures, and the client's needs are being met
  • Conduct evaluation and performance reviews of LPN and caregivers
  • Lead staff training programs and assures staff educational requirements are met
  • Addresses client and family concerns or complaints to the extent possible and follows up with appropriate staff to resolve the issue. Document complaints and report complaints to the Assisted Living Director
  • Educate residents about advance directives, medications, safety of side rails or assistive devices, potential hazards in the home and other relevant information
  • Assist with Quality Improvement as needed
  • On call Triage for medication errors/questions and resident falls or other incidents

Required Qualifications:

  • Active RN license
  • 1 year experience in an assisted living setting
  • Current knowledge of best practices for the elderly and residents with Dementia
  • Current Knowledge of best practices for medication and narcotics in assisted living setting and memory care
  • Experience with assessing and executing care of residents for assisted living and as they approach end of life care
  • Must be able to communicate effectively with staff, residents, families, community management officials, state regulatory agencies and agency staff, physicians, hospitals and the general public.
Desired Additional Qualifications:
  • Familiarity with Point-Click-Care highly desired
  • Previous experience in the Supervision of Care staff highly desired
  • Previous experience with On-Call for triage care staff support highly desired
