Immediate evaluations, visits, or 4-8-12 hour shifts available for in-home care or facility shift work.

Requirements to Register as an Independently contracted Caregiver:

• Valid Driver's License or Photo ID
• Social Security Card or Employment Tax ID Number
• RN or LPN License
• Level 2 background screening with ACHA fingerprints
• Physical within (6) months that states you are FREE OF COMMUNICABLE DISEASES (cannot be signed by a Chiropractor)
• CPR (American Heart Association, American Red Cross, or American Safety & Health Institute)
• HIV/AIDS certificate (minimum 1-hour)
• (LPN only) 30 hrs of IV infusion training (Optional)
• Previous Employment/ Contract history for the last 5 years
• Professional Liability Insurance (Strongly Suggested)
• Occupational Accident Insurance (Strongly Suggested)
