We are looking for a dedicated, compassionate RN to join our team as a Care Manager. Our Care Managers advocate for our clients, their families, and clinical staff to ensure the client is receiving the Highest Standard of Care.
Administrative Responsibilities:
- Onboard new clients
- Conduct client assessments
- Coordinate care for clients
- Monitor client satisfaction
- Supervise field staff
- Work independently outside of the office setting
- Participate in performance improvement
- Collaborate in client care meetings
Clinical Responsibilities:
- Providing hands-on care for clients
- Maintain accurate medical records
- Provide clear patient instructions and education
- Remote on call field and administrative support

- Proficient with EMR and Office Suite
Job Requirements:
- Minimum 3 years of nursing experience
- Current and unrestricted Colorado RN license
- Reliable vehicle / auto insurance
- Current BLS and negative TB test
- Clean background and criminal record
- Able to lift up to 50lbs
