VIP Home Health & Hospice Agency has an exciting opportunity for a full or part time RN Case manager. Job responsibilities include managing a patient caseload that includes both home health and hospice patients. Effectively communicating with patients, families, physicians and other disciplines to ensure a comprehensive approach to care. Participate in team meetings, accurately document patient progress and changes in patient's conditions. Submit required documentation in a complete and timely manner. Function as a team player within the interdisciplinary group of healthcare professionals.

This position requires on-call, is primarily Monday through Friday daylight hours, but may necessitate some evening and weekend hours. The successful candidate will have at least one year of previous experience in home health or hospice.
