• Demonstrate current knowledge of and adherence to state and governmental regulations and intermediary guidelines for home health care services.
• Demonstrates proficient understanding and adherence to continuous quality improvement objectives and safety, environmental or infection control standards.
• Responsible for current knowledge of patient’s conditions, needs, and responses to treatment. Immediately notifies Director of Nursing and physician of any changes in resident condition or response to treatments.
• Travel to patient homes and outreach locations to provide services to clients.
• Provides independent care to clients in remote locations.
• Responsible for continuing assigned RN Case Manager’s effective plan of care as prescribed by the physician; evaluates plan of care regularly and makes appropriate revisions as necessary and as approved by the physician. Communicates any changes to patient’s assigned RN Case Manager.
• Performs specialized nursing skills including professional assessments and direct patient care as prescribed by the patient’s Physician, Plan Of Care and within the State’s Scope of Practice for Professional Nursing.
• Initiates appropriate preventive and rehabilitative nursing procedures as necessary; obtains physician approval prior to changing client treatment and services
• Accurately and timely documents all nursing visits to client home and assessments on the day services were performed and on appropriate agency clinical forms.
• Delivers all documentation for patient charts to business office dailyResponsible for coordination and monitoring of patient services regularly; performs evaluations to determine effectiveness of treatments; make adjustments to care plans in collaboration with patient’s assigned RN Case Manager as necessary upon physician approval.
• Accurately develops and implements patient care plans in collaboration with health care providers, physicians, patients and families and other community services and agencies.
• Attends case conferences as scheduled to discuss and evaluate patient diagnoses, treatment plans, progress, goals, scheduling, problems, and interventions with interdisciplinary team; complete appropriate documentation of meeting to patient record
• Provides counsel and information to client and family members upon request regarding patient needs and services.

• Requires minimum of one year prior successful experience as a Registered Nurse. Home Health nursing care experience preferred.
• Must maintain current certificate in basic life support (CPR).
• Must provide proof of TB testing within the last 12 months upon hire or chest x ray.
• Requires travel to various geographic locations.
• Requires computer skills preferably Microsoft Outlook and Excel to accurately provide documentation on the iPad assigned.
