The RN/Nurse Trainer is a critical member of the Flat Rock Community Services team, led by the President/Chief Executive Officer who currently serves as the Administrator/Director of Operations (DOO) for Flat Rock Community Services.

Under the general supervision of the Administrator/DOO, the RN/Nurse Trainer monitors the on-going medical conditions of consumers served by the Supportive Living program and ensures the medical services provided by the Supportive Living team meets consumers’ needs and is provided in a healthy, safe, and compliant manner. This position may also monitor medical services provided by the Adult Programs team when consumers are participating in programs. The RN/Nurse Trainer also provides training for Flat Rock Community Services staff and ensures training certifications/recertifications are achieved in a timely manner. The RN/Nurse Trainer assists in medication audits conducted by the County Boards of Developmental Disabilities, coordinating directly with regulatory agencies on behalf of the Administrator/DOO. The RN/Nurse Trainer assists the Administrator/DOO in ensuring compliance for services within Medicaid’s and the Ohio Department of Developmental Disabilities’ rules for waiver settings and home and community-based services (HCBS) programs provided for supportive living consumers.

The RN/Nurse Trainer of Flat Rock Community Services will ensure medical records/binders, medication administration records (MARs) are maintained by staff in a compliant manner and will alert the Director of Supportive Living/Director of Adult Programs and Administrator/DOO to any concerns in this area. The position also ensures medication audits, other medical services compliance reviews, and required reports are timely and accurately completed/submitted. This position will ensure staff are adequately trained to provide the medical services they are employed to provide and receives additional medical services training as needed.

The RN/Nurse Trainer of Flat Rock Community Services ensures all medical services provided by Flat Rock Community Services are in compliance with Medicaid and state rules and guidelines. The RN/Nurse Trainer will assist in ensuring consumers’ rights and choices, by recommending and implementing approved policies, procedures, guidelines, processes, and checkpoints that ensure consumers’ rights and choices in delivery of medical services are honored to the extent possible. The RN/Nurse Trainer ensures coordination of delivery of medical services as outlined in the Individual Service Plan (ISP) and the consumer’s doctor’s orders and will assist in monitoring and then remedying Quality Assurance issues regarding medical services on behalf of Flat Rock Community Services.

The RN/Nurse trainer will help to cultivate an employee culture with high engagement leading to high levels of staff performance and a positive work environment through using effective and encouraging feedback, communication, correction (when needed), coaching, and training regarding medical services. In general, this position is regional and will rotate working at various Supportive Living homes; however, administrative work may be performed off-site/remotely. If desired, on-site office space can be offered at Flat Rock Care Center in Flat Rock, OH. Training will occur at the Flat Rock Care Center in Flat Rock, OH; at the Adult Day Support Program site in Bellevue, OH; at remote, rented sites in Lorain/Erie County; and/or at new program sites as they’re obtained.

This position is classified as “critical,” meaning that while some aspects of the work may be able to be done remotely, a substantial portion of the role must be done on-site at Supportive Living homes and program sites (training). Commuting to Supportive Living homes (currently Lorain County; Norwalk in Huron County; Huron in Erie County; and Flat Rock in Seneca County) will be required. Occasional travel for in-person events, meetings, conferences, pick-up/drop-off materials in Northern Ohio area may be required.

The RN/Nurse Trainer reports to Administrator/DOO for Flat Rock Community Services.

Responsibilities

The RN/Nurse Trainer of Flat Rock Community Services will hold the following responsibilities:

Medical Services Oversight, Coordination, and Compliance:

Oversight:

  • Provides oversight for medical services provided to by Flat Rock Community Services staff, ensuring quality of care and compliance of services provided to consumers. Activities may include overseeing, ensuring, providing, and/or completing:
  • Medical site visits/reviews for Supportive Living consumers, ensuring required documentation; medical orders/services; MARs; medical binders; pharmacy orders, etc. are complete, up to date, and accurate.
  • Ensuring medical services provided by Flat Rock Community Services Adult Programs staff is appropriate and required documentation is complete, up to date, and accurate.
  • Continual monitoring of health information and coaching/troubleshooting with staff to meet acute and chronic health concerns of consumers.
  • Frequent communication and coordination with doctor’s offices; hospitals; pharmacy; skilled nursing facilities; Flat Rock Care Center; therapy, home health, and other ancillary providers to ensure appropriate orders, documentation, training, etc. is in place for each consumer.
  • Creates, delivers, trains, and/or evaluates medical services to meet needs, ensure health and safety, and enhance the lives of Flat Rock Community Services consumers with disabilities within compliance with the Medicaid and state rules and guidelines for all services.
  • Assists in the investigation of Flat Rock Community Services’ Major Unusual Incidents (MUIs), Unusual Incidents (UIs); or allegations of abuse/neglect. Assists the Director of Supportive Living or Director of Adult Programs in ensuring all medical service issues are resolved and medical services contributing factors are remedied for the future. Ensures medical services documentation and reporting procedures are followed. Assists the responsible Coordinator/Supervisor or Qualified Intellectual Disabilities Professional (QIDP) in the investigation of injuries to consumers, known or unknown.
  • Assists the Administrator/DOO in ensuring Flat Rock Community Services compliance with other local, state, and federal laws, rules, and regulations as applicable for the provision of medical services.
  • Ensures and protects consumers’ rights and choices, by recommending policies and implementing effective procedures, guidelines, processes, and checkpoints that ensure consumers’ rights and choices are honored in the area of medical services.
  • Ensures coordination of delivery of medical services as outlined in medical orders and the Individual Service Plan (ISP). Monitors and remedies Quality Assurance issues related to medical services on behalf of Flat Rock Community Services. Advises Administrator/DOO of any potential issues that could adversely impact quality of medical care. Participates as a member of the Continuous Quality Improvement/Quality Assurance and Safety Teams.
  • Recommends appropriate medical services policies and procedures for Flat Rock Community Services operations within the framework of overall Flat Rock policy. May provide training on any Flat Rock Community Services policies and procedures.

Coordination:

This position collaborates with the Director of Supportive Living, Director of Adult Programs, Supportive Living staff, the Quality Assurance Specialist, and other RN/Nurse Trainers to ensure appropriate care and services are provided to each Flat Rock Community Services consumer.

Compliance:

  • Provides leadership in assisting the Administrator/DOO with Flat Rock Community Services’ compliance in medical services and training.
  • Serves as point of contact for all medication audits (or any other medical services audit) performed by the County Boards of Developmental Disabilities; Ohio Department of Developmental Disabilities; and/or Clearwater COG, representing the Administrator/DOO in these matters. Ensures all necessary information is provided to auditing staff in and efficient, timely manner. Keeps the Administrator/DOO updated on these audits.

Communication:

At all times, communicates positively and effectively with internal stakeholders (i.e., Flat Rock Community Services leadership team, Director of Adult Programs, Supportive Living staff, Adult Program staff, Quality Assurance Specialist, other Flat Rock staff, etc.) and external stakeholders (i.e., doctor’s offices; hospitals; pharmacy; Flat Rock Care Center; skilled nursing facilities; therapy, home health and other ancillary providers; County Boards of Developmental Disabilities; Service and Support Administrators; Clearwater COG; consumers; parents/guardians; and others) to ensure Flat Rock Community Services is viewed as a provider of creative solutions in the communities in which we serve.

Training:

  • Assists in the orientation of new Flat Rock Community Services staff by providing required and additional training to them.
  • Provides all mandated training as required for Flat Rock Community Services, including but not limited to CPR (American Heart Association); First Aid; and Medication Administration. Maintains certification to provide this training.
  • Regularly monitors required training and certifications to ensure all staff are recertified on a timely basis.
  • Provides additional training as required by Flat Rock Community Services.
  • Provides one-on-one or small group training on new medical services ordered for a consumer (i.e., oxygen, suppositories, enemas, etc.) to ensure staff feel comfortable providing new services.
  • Provides coaching to staff on any medical services issue with which they are having trouble or concerns.

Innovation/Sustainability:

  • Continually seeks innovative strategies to provide the highest quality medical services to Flat Rock Community Services consumers and training to staff.
  • Seeks ways to creatively use technology, volunteers, interns, and other methods to improve consumer outcomes and quality of medical care and services as well as to reduce burden on staff.

  • May provide occasional training to Flat Rock Care Center and/or Flat Rock Homes staff as needed.

Eligibility Requirements:

  • Must be able to meet any qualifications required for RNs and/or trainers as required by Medicaid and/or the Ohio Department of Developmental Disabilities.
  • Must have:
  • an active RN license in the state of Ohio;
  • One year of full-time (or equivalent amount of part-time) or more paid work experience as an RN (experience with individuals with intellectual/developmental disabilities preferred);
  • One year of full-time or more paid work experience in the supervision of staff.
  • Ability to read, write, and understand English at a level sufficient to comply with all requirements set forth in administrative rules governing the services provided by an agency provider;
  • Must be able to successfully pass a background investigation in accordance with rule 5123-2-02 of the Ohio Administrative Code and consent to be enrolled by the Ohio Department of Developmental Disabilities in the Ohio Attorney General’s retained applicant fingerprint database known as Rapback.
  • Must not have previously served as RN/Nurse Trainer/CEO/Director of Operations for any agency provider within the last five years that had its provider certification revoked/not renewed.
  • Preferred:
  • A bachelor’s degree in nursing;
  • Ability to read, write, and understand Spanish;
  • Ability to use American Sign language.

Remote connections with staff working in other locations will be required. Must have reliable internet service and the ability to use technology at home;
  • Must have experience using email, Microsoft Word, Microsoft Excel, Microsoft 365 or similar suite of products, and internet browsers.
  • Must have the ability to work a flexible schedule Monday through Friday. Willing to respond to emergency calls during evenings/weekends as needed.
