Position Overview: The Senior NRLP Long-Term Services and Supports Community Partnership Coordinator, is responsible for providing comprehensive and enhanced person-driven supports to enrollees, ages 3 to 64, that are participating in the program. The Senior LTSS CPC will also facilitate enrollee use of assistive technology, sustain enrolled engagement and encourage problem solving, manage data and supporting outcomes evaluations.

Position Responsibilities: Oversee management of resource database for all CPC's. Assist Program Manager with staff orientations and ongoing trainings. Cover caseloads as needed. Assist in the interviewing process for new Program staff. Back-up support for CPCs when Program Manager is out. Review information about the LTSS program to enrollee, ensure care plan uniquely represents enrollee, and update care plan regularly to reflect progress towards goals. Maintain regular contact with the enrollee that will include a face-to-face meeting in enrollee's home. Monitor the enrollee's health, functional, and social status.

Qualifications:

  • Bachelor's degree in Human Services or a related field or
  • An Associate's degree and three (3) years of previous work as LTSS CPC; Community Health Worker (CHW) or similar certification
  • Bilingual candidates will be given priority consideration
  • Previous work experience working with persons with disabilities a plus (children and adults)
  • Detail oriented
  • The ability to appropriately handle confidential and personal information
  • Proficient computer skills
  • Strong writing and communication skills
  • Reliable transportation and a valid driver license to make visits throughout the GLSS service area

Hours 40 hours per week, Monday through Friday
