Job Summary

Provide a multifaceted system of support services to the CTUIR elderly and their caregivers by assessing their needs and providing information on resources specific to aging, health, and related issues available. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

Essential Job Functions

  • Senior Caregiver Coordinator will be located at the Nixyáawii Senior Center.
  • Organizes monthly presentations at Senior Center of informative programs for elders.
  • Serves as a member of the 9 Oregon tribes Native Caring Conference committee.
  • Coordinate monthly Elders case management group meetings to report on Yellowhawk news related to seniors.
  • Organizes and maintains the materials in the resources library in an orderly fashion, stocks and orders materials in a timely manner.
  • Answers the telephone, greets individuals in person while using active listening skills to assess client needs. Assess needs of Elders/Caregiver/Grandparents raising children, facilitate referrals from other agencies or individuals and follows up with referrals to appropriate tribal, city, county or state resources that lead to assurance and empowerment.
  • Assist Elders/Caregivers/Grandparents raising children with accessing information and resources for respite care.
  • Networks with formal and informal service providers to increase quality and quantity of services for clients.
  • Keep confidential, accurate files, data and resources on Tribal Elders and their needs.
  • Maintains accurate, current records on usage of the Title VI grant and submits reports of services, data quarterly and timely manner for grant reporting periods.
  • Performs job-related clerical duties such as stuffing and addressing envelopes, preparing flyers, and collating materials.
  • Serves as the Senior Services Program representative in marketing and promotion of Senior Caregiving services at health fairs, tribal fairs, and other community outreach programs.
  • Maintain a quality assurance program that is designed to ensure high standards and provide safeguards by creating necessary policy and procedures to accomplish set standards.
  • Work with Supervisor to create budgetary and annual plan for Senior Caregiver Program.
  • Assist in preparation of reports for Supervisor, Yellowhawk Management and Health Commission.
  • Assist in the review and evaluation process of Senior Caregiver Program.
  • Available to travel outside the local area for meetings and training opportunities.
  • Maintain a positive working relationship with Yellowhawk, CTUIR, and Community Members.
  • Maintain confidentiality in accordance with Yellowhawk Personnel Policies and Procedures Manual, HIPPA, and the Privacy Act.
  • Be sensitive to patients, but maintain professional boundaries.
  • Must be capable of effectively working independently with minimal supervision.
  • Assisting in taking money for Senior Center meals, transport elders to obtain resources and meet forcase management (Medical transports not included).
  • Other duties as assigned.
