Job Summary:
Perform patient assessments, care plans and as applicable orchestrate discharge plans while
assisting the transition for patients and families to their new surroundings. Stay aware and
informed of privacy requirements and safety regulations according to both federal and state
guidelines. Ensures the emotional and social needs of the resident are met/maintained on an
individual basis as they relate to their medical condition/needs. Possess an attitude that
provides excellent customer service to all residents, families and staff while promoting the
company’s pillars of strength.

Responsibilities:
General Requirements
1) Complete MDS assessments timely and accurately
2) Complete documentation timely and accurately
3) Organize visits and maintain records for ancillary services
4) Assist with implementing and providing communications with hospice services
5) Participate in resident/facility rounds
6) Adhere to and enforce safety procedures
7) Report medical concerns or observations
8) Set up appropriate home health care upon patient discharge including but not limited to
follow up physician visits, therapy services and DME needs
9) Arrange necessary services for counseling or support services
10) Assist with psychiatric medication management
11) Be comfortable with one on one conversation, routine visits with residents, and
facilitating difficult conversations with families
Job Requirements:
1) Education: Minimum of bachelor’s degree, Master’s Degree preferred; must have a
social work license in the state of occupation
2) Experience: 1 year in healthcare setting preferred
3) Physical Demands: Lifting up to 50 pounds max.
4) Travel: Not applicable

5) Working Conditions: Be able to visit rooms with varying levels of light, smells and
temperatures as well as sit at a desk for prolonged periods of time making phone calls
and completing assessments
6) Principal Contacts: Residents, families and facility staff
7) General Requirements: Must be able to execute the terms and conditions set forth in
the employee handbook, including, but not limited to:
a) Works in a safety-conscious manner, which ensures that safe work practices are
used in order to not pose a risk to self or others in the workplace. Reports any safety
violations or hazards to supervisor
b) Complies with company policies and procedures and local, state, and federal
regulations
c) Adheres to policy on Drug-Free Workplace
d) Interacts in a tactful, diplomatic, and humanistic manner with patients, families,
visitors, and co-workers to provide a safe, efficient, and therapeutically effective
caring environment which ensures the self-respect, personal dignity, rights, and
physical safety of each patient and facility guest
e) Maintains a dependable attendance record and adheres to standards of cleanliness,
grooming, hygiene, and dress code
