Job Requirements:

• Provides direct social work services to Agency patients in their homes
• Works with the interdisciplinary team in integrating social work services into the plan of care
• Directs and assists in development and maintenance of resource manual utilized by Agency

• Ability to exercise independent judgement

• Excellent organizational and time management skills to meet accomplishments

• Can communicate clearly and effectively with care team

• Ability to understand and comply with Agency and regulatory standards of care

• Must have a high degree of flexibility and the ability to establish priorities

Education

• Master's Degree in Social Work from an accredited School of Social Work

Required Licensure/Certification

• Must have Maryland License in Social Work (LCSW-C) with counseling experience
• Must obtain current CPR certification
• Must have valid Maryland Driver's License
• Must have current auto insurance or reliable transportation

Work Experience

• Home Care experience is preferred
• Experience with management of adult population
• Clinical experience in counseling and casework for the terminally, chronically, and acutely ill adult and pediatric population
