This position is based at our central fill and mail order pharmacy in Yakima, WA.

The Clinical Specialty Pharmacist's primary responsibility is to ensure that patients receive safe and appropriate medication therapy. In this role, you are responsible for assisting the Pharmacy and Primary Care providers in the day-to-day management of medications, within the framework of legal guidelines and the professional Standards of Care. The Clinical Specialty Pharmacist provides pharmacist services for all of the Yakima Valley Farm Workers Clinic (YVFWC) pharmacies in Washington and Oregon, as needed.

Essential Functions/Responsibilities/Duties

  • Provide patient care services as assigned, which include but are not limited to:
    • Providing pharmacy services through relationship-based patient calls or virtual appointments
    • Supporting coordinated care at local clinics, including disease state management and population health management
    • Providing initial and follow-up Medication Therapy Management and patient education
    • Sitting on local or corporate committees, for example, Specialty Pharmacy Quarterly Management, Medical Management, or P & T
    • Prescribing a drug or device, as authorized by statute according to Drug Therapy Management Protocols
    • Ordering, interpreting, and monitoring laboratory tests
    • Receiving or transferring prescriptions
    • Drug Utilization Review
    • Drug Regimen Review
    • Patient counseling
    • Medication Therapy Management
    • Collaborative Drug Therapy Management, or other post-diagnostic disease state management, pursuant to a valid agreement
  • Ensure that all prescriptions, prescription refills, and drug orders are correctly dispensed according to the prescribing provider’s authorization and YVFWC policies and procedures.
  • Ensure the security of the pharmacy area, including providing adequate safeguards against theft or diversion of prescription drugs.
  • Ensure that all records and inventories are maintained in accordance with state and federal laws.
  • Oversee pharmacy personnel and students, ensuring that they work only within the scope of practice and duties of their licensure.
  • Maintain active licensure and pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing professional networks, and/or participating in professional societies.
  • Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA) and YVFWC requirements.

Qualifications

  • Education: Bachelor of Science Degree in Pharmacy; Pharm D Degree preferred.
  • Experience: 2 years’ experience working as a licensed Pharmacist, which can include a dispensing role in a community or retail pharmacy, and includes 1 year supervising a team in a clinical environment. PGY RESIDENCIES: The following residencies are the equivalent of work as a licensed Pharmacist: Post Graduate Year-1 (PGY-1) is the equivalent of 2 years, Post Graduate Year-2 (PGY-2) is the equivalent of 3 years. Experience with inventory and cost management activities preferred.
  • Licenses/Certificates/Registration: Current Washington State Pharmacist License and the ability to obtain an Oregon Pharmacist License.
  • Knowledge/Skills/Abilities Required: Knowledge of ordering and inventory control. Knowledge of cost management. Knowledge of community pharmacy practice. Current knowledge of State and Federal Pharmacy Laws. Knowledge of U.S. Food and Drug Administration (FDA) Health Regulations and the uses and effects of medications. Thorough understanding of dosage administration and measurement, chemical compounds and medical brands. Strong attention to detail and accuracy. Ability to successfully manage, coach and train staff for sustained performance. Effective verbal and listening communication skills with a customer-oriented approach. Ability to prioritize work and handle a variety of tasks simultaneously, with frequent interruptions. Proficiency with a variety of computer programs including Microsoft Outlook, Word, and Excel. Proficiency in one or more software systems such as Electronic Medical Record (EMR), Pharmacy Management, automated filling and/or Patient Communications Systems preferred. Ability to be credentialed and remain credentialed as a provider through the YVFWC credentialing committee or other means as determined appropriate by Sr. Pharmacy Leadership.
  • Drug testing: This position requires that candidates are tested for controlled substances before employment and will be subject to further testing throughout employment.
