PRIMARY RESPONSIBILITY:

1. Performs nursing assessments, as applicable per program.

2. Administers medications as prescribed by a physician order, following universal precautions.

3. Provides patient care as authorized by licensure and with minimal supervision in a prompt, efficient manner.

4. Acts as liaison between patient, clinical and medical personnel, and family when necessary.

5. Able to take direct and appropriate action in emergency situations, as authorized by licensure.

6. Utilizes appropriate safety measures in providing clinical care.

7. Reports any and all patient medical/behavioral concerns to supervisors, as soon as possible after occurrence.

8. Documents reportable occurrences accurately on an Incident Report and provide such to Program Administrator for review within 24 hours.

9. Follows chain of command and consults with supervisors on clinical and administrative on matters, when appropriate.

10. Adheres to all clinical protocols and policies and procedures of Citrus Health Network, Inc.

11. Develops and maintains care coordinator efforts with other CHN departments and community agencies regarding follow-up care.

12. Protects patients and employees by adhering to infection control policies and procedures, medication administration and storage procedures, and controlled substance regulations, as applicable per program.

13. Provides medication education to patient and family members, as needed.

14. Performs quality improvement monitoring activities in a prompt, accurate, efficient manner, as assigned.

15. Ensures all necessary medical supplies and medications are available, are not expired, and stored appropriate, as per regulations.

16. Works collaboratively with other program staff as a member of the patient care team

17. Must comply with requirements for EHR documentation based on policies and procedures including updates received via email, trainings, staff meetings, and other written communications.

18. Maintains safe patient care working environment by complying with procedures, rules and regulations with the assistance of health personnel.

MINIMUM REQUIREMENTS:

1. Must be licensed and maintain a current license in good standing with the State of Florida in accordance with Florida statute 464. One year of professional nursing experience preferred.

2. A diploma from an accredited college, school of nursing or university.

3. Proficiency in the use of computers, email and word processing software.
