Telehealth Navigator Job at Codman Square Health Center
Position Summary:
The Telehealth Hypertension Navigator provides outreach, support and health education to patients who have hypertension. These activities will occur in different locations and meeting formats. The goal is to be a connection between patients and their health care team as the patient gains skills and tools to manage their blood pressure. The Telehealth Hypertension Navigator is a member of the Codman Square Health Center’s Population Health Team, which is a dedicate to improving the health of the Codman community.
The role will allow the candidate to develop their own skills working in a multidisciplinary health care setting, including skills in addressing social determinants of health, responding to behavioral health needs, and implementing new technologies. The ability to be adaptable and learn new area of expertise is critical to this position.
Primary Responsibilities:
- Oversee outreach, enrollment, technology troubleshooting, device tracking, and data collection for patients enrolled in telehealth management of blood pressure through connected blood pressure devices.
- Responsible for maintaining registries of patients for outreach.
- Attend team meetings with other members of the Population Health team
- Screen patients for mental health and social determinants of health needs and be trained in response to identified needs
- The telehealth Navigator will be responsible for other duties as assigned
Qualifications and Skills:
- Education level: Associate degree preferred; Certified Nursing Assistant or Medical Assistant; two years of experience in health and/or mental health related field.
- Ability to work collaboratively with all care professionals.
- Multilingual in English, Spanish, Cape Verdean Creole, Portuguese and/or Haitian Creole preferred
- Experience working in community/health care setting.
- Competent computer skills, especially in Microsoft Office Programs (Word, Excel).
- Experience with Electronic Health Records, preferably Epic; or on the job training.
- Ability to learn about connected devices and remote patient monitoring to be able to assist patients.

- Must be able to stand or sit for prolonged periods (50% of the time) ·
- Must be able to lift 10+ pounds and load onto shelves
