This is a temporary 40 hours per week position for 6-9 months, fully onsite.
Monday-Friday between the hours of 8am-5pm.

We are looking for a dedicated Patient Navigator (PN) to help guide patients through various complexities of the healthcare system. This is a patient facing role, that will support our insurance and enrollment department through patient outreach, education, troubleshooting, and knowledge on all things pertaining to health insurance. The PN will provide continual support to patients and their families in need of health insurance enrollment assistance, education, renewals, and care continuity. Will be responsible for providing assistance and information to insured, underinsured and uninsured individuals about eligibility coverage and other financial assistance programs. The PN will provide a seamless patient experience and ensure that patients have a good understanding of their health plan and benefits and overall receive the needed assistance through health care transitions. Provide excellent customer service to all patient and staff in a diverse, multi-cultural and fast-paced environment.

Responsibilities include but are not limited to:

  • Educate and inform patients about benefits and responsibilities, the re-determination process and the importance of maintaining medical/dental coverage.
  • Provide information and resources to patients in order to make informed decisions about their healthcare
  • Conduct follow-up with patients to ensure that the process is complete and assist patients with enrolling into an appropriate health plan.
  • Complete training to become a Certified Navigator and recertify as needed
  • Review applications and required documentation to ensure complete and accurate submission
  • Verify patient information and respond to patient requests.
  • Work closely with the MassHealth Enrollment Center, Health Connector Office as well as the local hospitals.
  • Increase awareness about health insurance programs, health care and available services.
  • Make use of existing health center affiliations and resources as well as develop new methods of reaching new clients in the community.
  • Assist in the design and distribution of multilingual materials, including posters, display boards, educational brochures and other health related information.
  • Serve as spokesperson for the department and/or organization for multiple media venues pertaining to health care insurance enrollment.
  • Attend and participate in trainings, webinars and conference calls to stay current on state changes pertaining to health insurance.
  • Attend department meetings and trainings as needed to maintain current knowledge of registration processes, health center systems and changes.
  • Track and complete reports as needed, evaluate progress and revise strategies as appropriate.
  • Where needed, provide cross –coverage and back-up staffing support to other functions of the Patient Services Departments.
  • Perform other duties as assigned.

Requirements:

  • Insurance understanding and experience strongly preferred
  • Certified Navigators strong preferred
  • Strong customer services background required
