JOB SUMMARY:

A bilingual Virtual Care Nurse (VCN) provides support to the Complex Care Team Program at Concierge Home Care. A VCN participates and collaborates with the clinical team in the oversight of patients who are identified as high risk for hospitalization. This includes, but is not limited to, performing virtual visits, assessing psychosocial needs, monitoring telehealth, facilitating patient needs across the continuum of care, providing education in relation to the relevant medical diagnosis and factors that would increase the risk for hospitalization, and data tracking.

Education:

Licensed Practical Nurse (LPN)

Ex
perience:

Minimum of two (2) years’ experience as a home health clinician or health care professional, strong clinical background, and experience in triage/preventing patient hospitalizations is required. Experience with navigating patients through the healthcare system, virtual care platforms, comfort with navigation of electronic medical records programs and general office programs such as Microsoft Office is recommended.

  • Must have excellent problem-solving, critical thinking, and triage skills.
  • Ability to deal effectively and courteously with clients, family, physicians, medical professionals and co-workers.
  • Ability to interact and exhibit an empathetic attitude toward the sick and elderly.
  • Ability to communicate effectively, verbally and in writing.
  • Must maintain confidentiality of client records and information.
  • Knowledge of basic keyboarding, personal computer, and Microsoft Office.
  • Advocate for and provide information to patients and their families based on the client’s need.

Duties and Responsibilities:

  • Participate and collaborate with Concierge Liaisons on high risk readmission diagnosis and familiarize/research best practices and interventions to participate in readmission reductions. This includes; but is not limited to
  • Attending required rehospitalization related meetings, conferences, and symposiums throughout our agency and communities.
  • Tracking data, identifying hospital admission trends, and providing patient and staff education to prevent hospitalizations.
  • Assisting with innovation and implementation of protocols and guidelines for Concierge Best Practices Programs.
  • Manage virtual care and telehealth systems by providing virtual assessments, education, and tracking data.
  • Communicating patient information efficiently and swiftly to interdisciplinary team members, including community partners and providers.
  • Establish a trusting relationship with patients, caregivers, providers, and co-workers, based on a solid foundation of clinical knowledge.
  • Represent Concierge Home Care in a professional and positive manner while upholding the values and beliefs of the agency.
  • Comply with and support HIPAA Policies. Maintains confidentiality of information relating to the patient and family with those directly involved in the patient’s care.
  • As business needs may change, the duties and responsibilities may alter. In such event, the adjustment or addition of duties and responsibilities will be clearly identified and outlined by the assigning supervisor.

Physical Demands:

  • Occasional travel to Concierge Home Care offices, provider offices, hospitals, other relevant meetings, conferences, and/or patient’s homes.
  • Constant Sitting, Keyboarding and Fine Manipulation.
  • Constant Close Vision and Manual Dexterity.
  • Occasional Lifting up to 20 pounds.
  • Occasional Reaching Above Shoulder Level.

Conditions of Employment:

  • Must have a reliable internet connection in home setting (if working remotely)
  • Successful candidate must have a satisfactory post-offer competency and reference checks and must pass a pre-employment criminal background check.
  • Maintain a clean driving record and possess current auto insurance and Florida Driver’s License (or valid residency state license).
  • Maintain a clear clinical license and background check per company policy
  • Ability to work on-call on a rotating schedule for coordination of Complex Care Patient needs and triage with appropriate staff and providers.
