Job Summary

The Wellness Management Technician (WMT) is responsible for promoting the Dene' Model of Care to provide integrated support and care coordination for referred patients (un'ina). The WMT upholds the highest customer service standards, supporting both internal and external un'ina. The WMT greets un'ina, schedules un'ina for Wellness services and assists in Wellness activity support. This position will conduct and facilitate orientation for un'ina new to Wellness services, including orientation to Dena'ina Wellness Center (DWC) and other Tribal services.

Essential Functions

  • Provide excellent internal and external customer service by communicating in a clear and polite manner, demonstrating team work and providing comfort of un'ina waiting to see a provider
  • Work with team members to maintain Wellness referral queue (Fitness, Health and Wellness Coaching, Tobacco Cessation, Disease Prevention Classes, Health Support Groups, Traditional Healing)
  • Document follow-up communication via telephone, letters and electronic health record documentation
  • Re-schedule no-show and cancelled appointments
  • Ensure accurate data entry into Electronic Health Record, including un'ina communications, new demographic and insurance information, as well as purchased and referred care communication
  • Build bridges and create rapport with all team members within the organization and function as a liaison between specialty providers and un'ina
  • Provide un'ina education including gym orientation, DWC facility tour and explanation of services at DWC and the Tribe
  • Actively maintain population management database systems for Wellness classes and outreach activities to reflect current information
  • Facilitate warm transfer to Health System, Behavioral Health, Elders and/or Social Services when appropriate
  • Participate in procurement and distribution of Wellness supplies and reconcile departmental orders as assigned

Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.

Qualifications

Education

  • High School Diploma or GED or a combination of education and experience in a related field may be substituted for a degree

Experience

  • One year (1) of experience

License/Certification

  • Must have a valid Alaska State driver's license and must remain insurable under the Tribe's policy, or obtain within 90 days of hire, or, with approval of the hiring manager, sign a *Memorandum of Understanding (MOU) in lieu of having a driver's license to perform the required duties of the position (If Applicable)

Special Skills

  • Proficiency with Microsoft Suite, or obtain training within 90 days of hire
  • Ability to work independently as well as collaboratively
  • Conducts business in a service-oriented manner that is attentive, pleasant, respectful and kind when dealing with un'ina, visitors, co-workers and others
  • Memorandum of Understanding: Serves as documented evidence that the Kenaitze Indian Tribe has expressed the ineligibility of an employee to operate motor vehicles owned, leased or rented by the Kenaitze Indian Tribe, or to operate a personal vehicle on behalf of the Kenaitze Indian Tribe.
