About the Program: A community-based gun violence prevention program that aims to reduce and prevent shootings through the use of public health strategies. The program is a replication of the evidence-based Cure Violence model.

Job Summary: The Workforce Facilitator will provide work readiness and related skills training and may also provide training toward attainment of foundational occupational credentials.

Essential Functions

• Provide case management counseling to assist youth in meeting vocational and educational goals
• Carry a caseload of up to 15 youth that are high risk for gun violence to assist with identifying and achieving obtainable goals
• Conduct assessments and refer youth to education, vocation, mental health, substance abuse or any other identified service need
• Completes paperwork for youth in a timely manner including but not limited to case notes, assessments and referral materials
• Assist youth to create a portfolio i.e.; resume, cover letter, a list of references, copies of vital documents such as birth certificate, social security card, etc.
• Develop appropriate career exploration activities, including work readiness and career coaching
• Provide education on employment soft skills, including interview techniques and strategies, importance of arriving on time for work, what to wear, and respectful communication with a supervisor
• Act as a source of support for youth and provide appropriate feedback to promote youth success
• Assist youth in obtaining and maintaining employment that is consistent with their individual vocational goals
• Assess youth’s vocational functioning on an ongoing basis utilizing background information and work experiences
• Document all interactions and meetings in progress notes
• Other duties that may be assigned

Qualifications

• High School Equivalency required; A bachelor’s degree from an accredited college preferred
• Two years of experience in community work or community centered activities in an area related to employment or education development
• Working knowledge of Microsoft Office Suite, including Excel
• Must be a self-starter and able to work independently and in teams on projects
• Excellent communication, planning, organizational and multi-tasking abilities.
• Prior experience with case management and facilitating groups preferred
