ESSENTIAL FUNCTIONS:

  • Demonstrates competency in providing education to patients, staff, and the community on ACP
    • Initiates and runs sessions for staff, patients, and the community
    • Ability to provide education/support to community groups
    • Develops written and verbal education for patients/families
    • Initiate completion of directives with patients, family, and staff
    • Provides training and education to hospital staff.
    • Is a resource for staff and provides onsite training
    • Organizes National Decision Day and activities for the organization
  • Demonstrates ability to meet identified standards to support the ACP conversation throughout our organization and in the community
    • Establish metrics to outline the success of the program and identify areas of opportunity
    • Maintains notary list for those employees who will notarize advance directives
    • Create programs to increase ACP conversations for the community, our pts and staff
    • Branches efforts with our WHP primary care offices to support ACP in the offices
    • Create a process to track referrals and completion of directives throughout the organization
  • Demonstrates competency in effective communication.
    • Demonstrates the ability to communicate with persons from other cultural groups.
    • Collaborates with the Communications Dept on ensuring the development of education materials and advance directive booklets in needed languages
    • Recognizes and respects diversity of co-workers and patients
    • Demonstrates the ability to communicate with persons from other cultural groups
    • Keeps necessary staff informed of high risk/problematic cases
    • Establishes processes to help streamline the ACP conversation for staff
  • Demonstrates competency in the performance of duties and responsibilities related to organizational skills, boundaries, and collaboration.
    • Work is completed in a timely manner.
    • Triage workload appropriately.
    • Offers assistance to coworkers when able.
    • Information sharing is done in a timely and organized manner.
    • Demonstrates collaboration with internal and external partners

Qualifications


Experience Minimum Required

  • One year in healthcare setting

Experience Preferred/Desired

  • Two years’ experience including recent experience in acute health care setting

Education Minimum Required

  • MSW or ADN degree

Education Preferred

  • BSN and Licensure

Training Minimum Required

  • Advanced Care planning certification

Special Skills Minimum Required

  • Knowledge of medical terminology.
  • Ability to organize time and establish priorities; ability to communicate in written, electronic, and verbal formats; ability to give and receive information over the telephone; ability to make effective judgments and decisions based on objective criteria.
  • Ability to work in a stressful environment and handle sensitive issues; ability to maintain confidentiality; calm, reassuring, empathetic demeanor; flexibility, professional in interaction with people at all levels; ability to work with a wide range of patient conditions, personalities, and cultures; ability to provide good internal and external customer service.
  • Ability to operate a computer.
