Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Essential Duties:

• Promotes a welcoming, positive customer service culture which encourages quality patient outcomes and promotes staff retention

• Assists in performing comprehensive psychosocial assessments with patients in order to determine appropriate level of care and discharge planning needs

• Conducts informational/educational sessions providing appropriate materials to educate patients on addiction recovery and aftercare options

• Acts as coordinator for discharge needs of assigned patients

• Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed

• Networks with others to obtain other resources for patients as needed

• Works collaboratively in formulating an appropriate and realistic discharge care plan, communicating with the patient

• Completes appropriate forms and contracts all appropriate individuals to facilitate the transfer/linkage of patients to other community-based agencies and services.

• Ensures all referrals are linked through follow-up

• Documents all relevant clinical information pertaining to patients based on assessment, education, and discharge planning activity

• Prioritizes and organizes work according to the acuteness of the situations presented

• Maintains good working relationships with hospital staff

• Other duties assigned

Schedule: Monday - Friday (8:00a - 4:30p) with flexibility to meet patient and agency needs

Required Experience:

Education and/or Experience: High School Diploma or GEDrequired; Associate Degree (or higher) in Psychology, Counseling, Social Work or a related field are encouraged to apply

Certification, Licensure, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required or able to obtain within two (2) years of hire date
