Blended Case Manager

Hours: 40 hours per week, 52 weeks per year

General Description of Duties:

Blended Case Management provides case management services to adults and children who suffer from a serious mental illness or emotional disturbance. An assigned case manager works with the individual and families to access, link, coordinate and monitor needed services as well as provide support and advocacy. The primary goal of the Case Manager is to maintain the individual in the home and community settings. This is a non-therapist position

Qualifications-Education:

• Bachelor’s Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education required;
• or be a registered nurse;
• or a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years’ experience with direct contact with mental health individual.

Additional Qualifications:

• One to three years working with children and/or adults in mental health setting and one to three years of case management experience preferred.
• Must have audio/visual/verbal acuity to perform essential job functions.
• Must have physical dexterity to work in individuals’ homes and in the community doing activities including walking, lifting, cleaning, etc.
• Must possess excellent verbal and written communication skills.
• A valid driver’s license and access to a reliable automobile is required.
• Must be able to adapt hours to needs of position. May need to work evenings and/or weekend hours periodically including on-call emergency services on a rotating basis.
• Must be able to remain organized despite pressured situations.
