Responsibilities/Activities:

• Duties vary depending on the individual client’s Service Plan. They can include all, or a selection of, the following activities although other authorized activities could be added to meet a specific client’s needs
• Provide companionship, friendship and emotional support.
• Provide transportation to medical appointments, grocery store and errands.
• Teach/perform meal planning and preparation.
• Driving for medical appointments/grocery shopping
• Performing bathing/showering/sponge baths, skin care, personal hygiene, toileting (including bladder & bowel care),
• Perform light housekeeping/laundry
• Ensure service is delivered in accordance with Agency policies, procedures and industry standards.
• Assist in basic client transfers providing the client has been assessed as being capable of ambulating without assistance; and/or, providing another trained caregiver (including family) is involved in the transfer.
• Observe the client’s functioning and report to Supervisor.
• Complete and maintain records of daily activities, observations, and direct hours of service.
• Observe Client and their environment. Report unusual behavior, physical/cognitive changes and any changes in living arrangements to Agency Manager.
• Make sound decisions. In case of emergency call 9-1-1 first, then notify the Agency Manager.
• Attend orientation, in-service training sessions and staff meetings.

Required Knowledge:

• Knowledge of principles and processes for providing client services, including needs determinants, meeting quality standards and evaluation of client satisfaction.
• Knowledge of the English language.
• Knowledge of safeguarding matters and how to recognize and report abuse through incident reports.
• Knowledge of health and safety matters relating to home care and ensure client’s safety.

Required Skills/Abilities:

• The ability to provide care in a manner that respects client demographics, cultures or beliefs, ethnicity, and race.
• The ability to be aware of other people’s reactions and understand why they react as they do.
• The ability to establishing and maintain relationships.
• The ability to teach others, listen actively; communicate information orally and in writing, with the ability to follow orders and instructions.
• The ability to identify problems and determine effective solutions
• The ability to apply reason and logic to identify strengths and weaknesses of possible solutions.
• The ability to work independently and in cooperation with others.
• The ability to establish and maintain harmonious relations with clients/families/co-workers.

Qualifications/Education:

• HS Diploma
• Current driver’s license and proper Vehicle Insurance Coverage.

Training/Experience:

• Caregiver training certificate
• Meets training requirements of Illinois
• One or more years of caregiver experience
