Position Summary:
We are currently looking for a Case Manager to serve our Onondaga and Oswego Kinship communities~

In the Family Services division, we provide clinical, skill-building, independent-living, foster-care, and case-management services for children who are at risk—so that they can get the level of care and support they deserve. Our Kinship Care program is designed to provide outreach, community education, respite, family engagement activities, support groups, referral, client assistance and case management services for nonparent caregivers of children who are related by blood or a prior relationship.
Under the direct supervision of the Program Supervisor, the Case Manager will provide services to clients in the Oswego and Onondaga areas and will provide trauma-informed case management services to kinship clients in order to:
  • Increase knowledge of permanency options
  • Improve parenting strategies.
  • Provide for the well-being, safety and stability of kinship children.
  • Facilitate training and support groups for kinship families.
  • Increase community connections for kinship families by establishing relationships with DSS and community service providers.
  • Complete screening and assessment of kinship families.
Qualifications:
  • Bachelor’s degree in social work or related degree and at least two years’ experience providing services to children/families.
  • Must possess a valid New York State driver's license and access to reliable transportation.
