Tri-County CAP is a multi-program non-profit agency that is dedicated to improving the lives and well-being of New Hampshire’s people and communities. The goal of our Homeless Intervention/Prevention program is to alleviate immediate housing emergencies and to assist shelter residents with achieving independence that leads to permanent housing and ends the cycle of homelessness.

The Homeless Intervention and Prevention Specialist – Case Manager for Permanent Supportive Housing position will provide CTI case management services to those who are homeless or at-risk of homelessness in Coos, Berlin, and Grafton County in an effort to assist and engage those experiencing a housing crisis with securing and maintaining permanent housing. This is a full time, 40-hour a week position.

Responsibilities:
 Work closely with the Homeless Intervention and Prevention Specialist – Outreach staff
to assess the individual or family need related to rental assistance or security deposit
loan eligibility.
 Support with completing applications for rental assistance and/or security deposit loan in
order to obtain or re-stabilize permanent housing.
 Work closely with Outreach staff and local landlords to obtain permanent housing for
clients.
 Work closely with the client and Housing Stabilization Coordinator for timely processing
of rental assistance applications.
 Utilize CTI case management to address barriers to stable housing and support with
making meaningful and long-lasting connections to community programs and social
services.
 Determine type and availability of resources needed to meet individual needs of clients.
 Provide support in obtaining essential services within the community such as food
stamps, mental health referrals, Medicaid, transportation, and disability services.
 Serve as an advocate for clients by acting as a liaison between them and community
programs, potential landlords, and other social services.
 Support with maintaining housing by acting as a mediator between the client and
landlord.
 Provide tenancy education to clients on ways to be a desirable tenant and legal rights as
a tenant.
 Create action plans with clients utilizing SMART goals, organize files, document client
progress and enter data into EmpowOR database in a timely manner.
 Provide support in obtaining basics such as housing, healthcare, and education.
 Attend scheduled quarterly ESG meetings and other meetings to stay up to date on
current trends and regulations as requested by supervisor
 Represent TCCAP Homeless Intervention and Prevention Programs at local networking
meetings and other meetings as requested by supervisor.
 Work closely with various local town welfare offices and human services agencies as
needed.
 Support with fundraising efforts and securing municipal funds by representing TCCAP’s
Homeless Intervention and Prevention Programs within the community and at town
meetings as requested by supervisor.
 Other duties as assigned by Homeless Program Manager.

Qualifications, Skills and Experience:
· High level of interpersonal skills to handle client services situations with tact.
· Ability to prioritize, problem solve and take action as appropriate.
· Experience with crisis intervention
· Knowledge of local and community mainstream resources.
· Demonstrated ability to be self-directed and motivated with minimal supervision.
· Knowledge of program services, client needs and management techniques, needs of client populations and treatment sources.
· Understanding of homelessness and experience working with those with a history of or experiencing trauma, mental illness and/or substance abuse disorders.
· Skills in case management including an understanding of professional boundaries.
· Skills in prevention and diversion in an effort to reduce homelessness.
· Basic understanding of Microsoft Office Suite.
· Frequent travel is expected, valid Driver’s License a must
