We are looking for a Case Manager to join our Emergency Service Team.

Jewish Family Service Agency Emergency Services provides nondenominational, criteria based social resources to those who need assistance in these areas: Transportation, Emergency Assistance, Nutrition, and Housing with an emphasis on stability and self-sufficiency.

The Emergency Service Intensive Case Manager will work directly with homeless clients to create a professional case management plan and perform related duties as required to meet program guidelines.

The Emergency Services Intensive Case Manager should be energetic, and passionate about providing quality services to clients in hardship situations, including the homeless.

Tasks/Responsibilities

  • Conduct client assessment to determine client needs and set up a case plan to be followed.
  • Provide in home intensive case management to program eligible individuals
  • Complete all paperwork in a timely manner related to and for clients being assisted.
  • Provide appropriate resources, tools, counseling to assist participants in achieving their case plan goals. Making referrals for services, following up, tracking and documenting participant’s progress.
  • Maintain positive relations with clients, local community agencies and team members.
  • Document all client contact, maintain client files, and document all information
  • Be an active and collaborative member of the Emergency Services team at JFSA
  • Participate in various committee meetings and trainings related to community resources.
  • Maintain client confidentiality.
  • Report client concerns/ needs to Director in a timely manner
  • Maintain compliance with program guidelines.
  • Perform other duties as assigned by the director and as needed for the programs.

Job Type: Full-time

Pay: From $47,500.00 per year
