POSITION TITLE: Drop-In Case Manager

RESPONSIBLE TO: Youth Services Coordinator

RESPONSIBLE FOR: Youth Outreach Drop-In Center is a place for at-risk youth to access services. Services include recreational activities, skill-building groups, job/career development, academic support, leadership opportunities, shelter, supply distribution (food, clothes, hygiene products) and other services for at-risk youth, with a special emphasis on runaway/homeless youth. The Drop-In Case Manager general responsibilities are: 1) Youth case management as assigned by Supervisor. 2) Responsible for supervision of youth as well as the program development and implementation of activities and small groups for youth who visit the Center; Schedule is Monday- Friday and 11am to 8pm. Occasional evenings and weekends may be required.

ESSENTIAL DUTIES

  • Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.
  • Provide a safe, nurturing, and well-supervised place for youth age 11-21 to participate in and access services.
  • Interact with and supervise youth who participate in Youth Outreach Drop-In Center activities.
  • Work with staff to monitor youth behavior, enforce behavioral expectations, and utilize appropriate discipline measures, as necessary.
  • Provide and demonstrate positive role modeling.
  • Daily facility maintenance (putting away supplies, sanitization, setting up space for appropriate use) and cleaning.

· Conduct comprehensive intake, assessment, and service plans for at-risk youth who visit the Drop-In Center.

· Provide on-going support for at-risk youth including developing and monitoring client goals, advocacy, referrals to community resources, and outreach.

  • Refer youth to appropriate community resources, provide follow-up monitoring on placements or referrals.
  • Develop and implement quality age-appropriate skill groups, activities, and programs for the Drop-In Center on a regular basis.

· Collaborate with other staff, agencies, businesses and volunteers to ensure that agencies’ programs coordinate to provide the best possible services for youth and wise use of resources.

  • Maintain current documentation and records as required for program evaluation and reporting purposes and ensure all files and reports are accurate and meet grant standards.

· Serve as “after-hours” on-call staff as needed to ensure participant monitoring and timely crisis response.

  • Support street and school outreach efforts and activities as part of an outreach team.
  • Drive to and from locations for outreach, and transport youth to and from school, appointments, or other essential needs.
  • Other duties as assigned by Supervisor.

KNOWLEDGE, SKILLS AND ABILITIES

  • High School Degree/Some college studies
  • Preference given to college work in Social Work or related field
  • Strong interpersonal skills for connecting and working with youth from diverse backgrounds (including, but not limited to, ethnic, religious, physical, and mental abilities, etc.)

· Demonstrated ability to help others set and follow through with goals, preference give to Case Management experience

· Experience in providing direct services to at-risk youth, ages 11-21 and their families

· An understanding of at-risk, runaway, homeless, and street youth issues

· Bilingual- (English and Spanish) and bi-cultural experience and abilities preferred

· Recognition of appropriate boundaries and the ability to implement them

· Ability to work effectively in stressful or crisis situation

· Commitment to empower youth to make informed decisions

· Ability and willingness to work under pressure

  • Strong oral and written communication skills
  • Commitment to work as an essential part of a team
  • Demonstrated ability in the implementation and development of quality youth programing
  • Computer experience
  • Ability to be flexible in work schedule
  • Ability to pass drug/alcohol & background check and insurance check of driving record

PHYSICAL DEMANDS

The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee may be required to sit, stand, kneel, walk and/or bend for extended periods of time. May lift and/or move up to 20 pounds.

Job Type: Full-time

Pay: $18.06 - $19.50 per hour
