Summary/Objective

The Center Monitor function is to ensure that there is adequate coverage for New Vista Hazelwood, Bingham Gardens as well as the community homes. The Center Monitor will also ensure the safety and welfare of the residents who reside at New Vista Hazelwood and Bingham Gardens through enhanced monitoring. The Center Monitor functions in a collaborative relationship to assist with administrative issues that may arise at the facility. All services are provided at the highest standard of professional practice, complying with standards by both federal and state regulations and facility policies.

Essential Functions

Under the supervision of the Shift Coordinator Supervisor:

• Ensure there is adequate coverage for the facility.
• Ensure the safety and welfare of clients by making rounds across the facilities and maintain high visibility in the homes and work areas of clients and staff.
• Provide observation, interaction, and support for staff to ensure they are alert and conducting client care in a professional, caring, and compassionate manner.
• Complete and distribute appropriate documentation, including but not limited to facility coverage and Enrichment center assignments.
• Take immediate action regarding any issue found.
• Assist with Administrative issues.
• Work with other departments to assist with getting necessary materials, information or documents that may be needed.
• Ensures the overtime staff roster is maintained and updated daily.
• Review and report all incidents in the facility.
• Collaborate with the Facility Director’s Office and the AOD on reporting incidents.

Supervisory Responsibility

Oversees operations of the Facility including but not limited to staff distribution, staff responsibility, Risk Management / Incident report completion.

Position Type/Expected Hours of Work

Full Time: Full time 37.5 hours per week: schedule will include weekends, holidays, and overnight shifts. 12 hours shifts. Must be able to flex hours and work overtime as needed.

This position does not require travel.

Required Education and Experience

• High School degree or equivalent
• Two years of experience working with individuals with intellectual disabilities.
