This individual will be responsible for providing individual support and skills development to individuals in the community. This position promotes meaningful connections and roles in the community as well as assist individuals to gain access to needed behavioral health, medical, social, and residential services.

Responsibilities:

• Work directly with individuals to asses their strengths and interests.
• Link individuals to community resources and peer supports to promote community integration.
• Participates in the analysis and identification of service delivery gaps, changes and problems under the guidance of the supervisor.
• Provides and facilitates individual skill development and support.
• Participates in the resolution of crisis situations and identify individual with resources that can provide direct support.
• Advocate for and encourage individuals to assume a proactive role in their recovery process.
• Attends staff and other meetings as it relates to individual's overall growth and development.
• Assists with special projects and program planning.

Education/ Experience

• Bachelor's degree in social work, psychology, sociology or a related field is required.
• Two years of experience working in the mental health direct service, one of which must be in Psychiatric Rehabilitation Services (PRS).
• Certified Psychiatric Rehabilitation Practitioner (CPRP) required.
