What is a Child Youth Mentor and Direct Support Provider?

What is a Direct Support Provider? A Direct Support Provider is an employee or contractor that will develop a relationship with their client in a caring work environment while following PPRS and State of Colorado guidelines for care. This care could include Respite, Homemaker, Community Connector services, Mentorship or Host Home opportunities.

• Qualifications:
• High School Diploma or Equivalent
• Experience with Developmental Disabilities is preferred but not required
• Professionalism
• Focus on individualized care and support
• Ability to communicate with parents/guardians and admin staff.
• Good written and verbal communication skills
• Valid driver's license and clean motor vehicle record (if providing services with transportation)
• Clear background screening and reference checks
• Ability to chart individual progress through a web based system

What is a Child Youth Mentor? A Child Youth Mentor is an employee or contractor that is part of a “team” for a family needing services. The team is generally involved in the wraparound process and can include a myriad of services and responsibilities. To include but not limited to crisis situations, (stabilization, prevention, de-escalation), interventions, collaboration with schools or therapy, and supports for the child/youth and their family to prevent out of home placement. The Child Youth Mentor will work closely with the parents/guardians as well as the Facilitator from PPRS to provide services for the dynamic needs of the individual.

A Child Youth Mentor will develop a trusting and communicative relationship with the individual, provide advice and mentorship conversations, find safe and fun activities in the community to create learning and integration moments, be an advocate, listener, and coach.

Qualifications:

• High School Diploma or Equivalent

• Understanding of Service Plans and Individualized Support Plans
• Focus on individualized care and support
