Chronic Care Nurse Coordinator:

The Chronic Care Nurse Coordinator is responsible, managing the transition of and outreach to medically complex patients and facilitating consistent team meetings that support team development, teamwork, and new workflow training.

Monday – Friday 8:00a-4:30p

Responsibilities:

• Contributes to the actualization of the vision philosophy and critical success factors of the Department of Nursing
• Commits to maintain the standard of the practice of nursing and are responsible for the coordination, implementation, and delivery of care to the neonate, pediatrics, adolescent, adult and/or elderly patients within their scope of practice
• Participates in performance improvement, research activities and maintains clinical expertise
• Coordinates the continuum of medical care services for patients to ensure optimum utilization of resources, delivery of quality patientcare, and compliance with regulatory, accrediting, and contractual requirements
• Determines the appropriateness of patient care services, prepares clinical appeals for third party denials, and collects and interprets data for utilization, compliance, QI, and other areas as assigned

Requirements:

• NY State License for Registered Nurse and current registration
• BSN Required, MSN preferred
• Certification in specialty preferred
• Effective English communication skills, written, oral, and interpersonal
• Ability to use basic Excel data entry and retrieval system
