Sheltering Arms Institute is seeking a Clinical Coordinator to join the team. The Clinical Coordinator facilitates patient flow, and coordinates activities and communication for the nursing unit during his/her assigned shift. This person ensures that patient care is delivered to all patients on the nursing unit in an efficient, caring, and courteous manner.


Performance Responsibilities

Transdisciplinary Care

  • Ensures all team members are oriented to the transdisciplinary culture and expectations on the unit. Clearly communicates the structure, practice, and expectations of the Sheltering Arms Institute care model.
  • Provides support and coaching to team members to help them transition into and maintain a philosophy of transdisciplinary teamwork through role release and maintaining the highest standards of patient satisfaction.
  • Models transdisciplinary behavior and leadership, assists in setting unit expectations regarding behavior, assists families and patients with their needs to foster a more collaborative, meaningful, and engaged patient care experience.
  • Facilitates access to and engagement with formal and informal interprofessional education for all team members, including learners.

Continuing Education

Pertains to Team Members in Licensed Nursing Positions ONLY:

  • Team member must complete 15 contact hours of Continuing Nursing Education during the evaluation cycle.

Nursing Process

The Clinical Coordinator demonstrates competent use of the nursing process as a problem-solving model in performance of job duties as evidenced by:

  • Demonstrating expertise in the nursing process by drawing from depth and scope of knowledge; displaying documentation and performance that reflects the process.
  • Focusing on the overall patient needs of the unit and assesses for patient/staff needing assistance, verbal or nonverbal behaviors that indicate patient discomfort, pain, distress or dissatisfaction.
  • Assisting with prioritization of activities for patients on the unit.
  • Monitoring the flow of the area/unit for consistency of care and smooth functioning.
  • Ensuring timely admission and discharge process to ensure that patient care is based on thorough nursing process and that hospital capacity is maximized.
  • Demonstrating expert assessment skills.
  • Taking appropriate actions to enhance positive outcomes, even in the most challenging situations.

Nurse/Patient Relationship

  • The Clinical Coordinator reinforces and promotes a holistic approach in preserving a therapeutic nurse/patient relationship throughout the health care continuum as evidenced by:
  • Communicating with patients to resolve questions or concerns.
  • Modeling appropriate response to patient/family concerns.
  • Ensuring plans of care are shared with the patients.
  • Demonstrating respect for colleague’s relationships with their patients.
  • Consistently demonstrating effective communication with members of the team.
  • Empowering staff to assist patients throughout the health care continuum.
  • Promoting holistic, non-judgmental care.
  • Targeting the overall patient needs of the unit and assessing for patients/staff needing assistance.
  • Observing for verbal or nonverbal behaviors that indicate patient discomfort, pain, distress or dissatisfaction.

Assuring Quality of Care

The Clinical Coordinator assures quality of care as evidenced by:

  • Valuing the patient/family rights to receive safe, cost-effective care and facilitates the process.
  • Ensuring adequate resources for patient care.
  • Investigating narcotic discrepancies and incident reports.
  • Collaborating with Bed Management in the appropriate and timely placement of patients.
  • Identifying potential safety hazards on the unit and begins resolution.
  • Making sure assignments encompass quality control and environment of care standards.
  • Embracing TJC and Patient Safety Goals into personal practice and the practice of the nursing team.
  • Anticipating staff needs to provide quality and efficient care.
  • Resolving bed flow issues and follows through on identified deficiencies in process.
  • Adapting systems and situations to promote high level, cost effective care.
  • Supporting nursing staff by going into staffing when needed to provide quality of care and patient safety.
  • Serving as a role model by supporting coworkers in seeking evidence-based practices that would improve the quality of care for the patients.

Education

The Clinical Coordinator maintains an environment that is conducive to positive learning experiences for students and healthcare professionals as evidenced by:

  • Serving as a clinical resource and preceptor/mentor for new staff.
  • Mentoring RN staff orienting to the charge nurse role.
  • Working with the Nurse Clinician to ensure the educational requirements and clinical proficiency are maintained for staff.
  • Is committed to continuous learning through educational and professional improvement programs.
  • Creating an environment that promotes educational growth opportunities for nursing staff (i.e. PAP or specialty certification)
  • Encouraging staff to initiate research projects based on Evidenced Based Practices.
  • Anticipating staff, patient, and family learning needs.

Leadership/Collaboration & Professional Development

The Clinical Coordinator demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care as evidenced by:

  • Communicating issues to and seeks guidance in resolving problems from the Nurse Manager, House Supervisor, Director of Nursing or other appropriate administrative leader in a timely manner.
  • Handling complaints from patients, families and other departments and refers unresolved issues to the Nurse Manager as appropriate.
  • Makes assignments, coordinates breaks, and assigns meal times.
  • Ensuring assignments are appropriately delegated with consideration of patients’ needs, competencies of staff, geographical layout of the unit, and workload balance.
  • Facilitating a cooperative, team relationship with therapy, support services and ancillary departments.
  • Reinforcing a climate of collaboration and mutual respect among caregivers and between staff, patients, and families.
  • Ensuring patient admissions and discharges are managed expeditiously.
  • Participate in annual performance review.

Performs other duties and participates in special projects

  • Performs other duties as assigned and/or participates in special projects in order to support the mission of Sheltering Arms Institute and departments.
  • Provides assistance to team members.
  • Accepts alternate assignments as required.

Qualifications and Skills

  • 3 years previous experience providing care in either an acute care or inpatient rehab facility required.
  • Charge nurse or supervisory experience.
  • Experience with staffing, scheduling and skill mix, preferred.

Education/Certifications

  • Graduate of an approved nursing program (RN), bachelor’s degree in Nursing, preferred.
  • RN - Licensed Registered Nurse, in good standing
  • AHA BLS HCP Certification or equivalent
  • Certified Rehabilitation Registered Nurse (CRRN), Certified Brain Injury Specialist (CBIS) or other nursing specialty certification or willingness to acquire within defined parameters, preferred
