Job Summary*
Provides assistance to the Director of Patient Care Services and Clinical Field Staff to ensure provision of quality patient care and effective team performance to meet company goals in accordance with company’s philosophy, policies, mission, and vision, and prescribed local, state, and federal regulations.

Responsibilities*

Ensures that upon admission to Lorian all patients are thoroughly assessed and that based on that assessment the appropriate clinical services are rendered to the patient using Lorian’s interdisciplinary team approach. This includes, but will not be limited to:

  • Performs initial care coordination and subsequent evaluation of plan of care.
  • Ascertains proper mix of services (i.e. SN, PT, OT, ST, HHA, RD, and/or MSW).
  • Determines adequacy of proposed visit frequencies given patient’s acuity and plan of care (i.e. A=< 9 visits, B=< 15 visits, C=<20 visits, etc.).
  • Ascertain optimum clinical coverage (e.g. LVN vs. RN, OT vs. HHA, etc.).
  • Evaluates clinical practices of specified disciplines through the review of clinical records to ensure that standards of care are met and quality services are provided in accordance with prescribed Lorian’s, local, State, and Federal guidelines. This includes, but will not be limited to:
  • Reviews visit notes to ensure that plan of care is being properly executed, planned visit frequency is being adhered to, and care goals and objectives being assessed, measured, and accomplished on an on-going basis.
  • Reviews patient charts to ensure that wound care orders are being properly executed and wound pictures are taken.
  • Reviews patient charts to ensure that interdisciplinary care coordination/communication is taking place.
  • Coordinates staffing patterns to optimize utilization of clinical staff while at the same time meeting patient needs and the needs of the interdisciplinary geographical clinical teams. This includes, but will not be limited to:
  • Reviews intervention by different disciplines and minimizes duplication of effort and overlapping of services (i.e. exhaust OT intervention prior to referring HHA).
  • Reviews patient’s address and assigns patient to clinicians respective of their territory.
  • Works with the Human Resources Department in anticipating and recruiting clinicians in areas of need.
  • Collects and analyzes staff visit data to ensure that individual clinicians are consistently meeting prescribed productivity standards. This includes, but will not be limited to:
  • Works with DOPCS and PCC Team to ensure that individual clinicians are consistently assigned the proper number of daily visits.
  • Facilitates the team process by promoting interdisciplinary communication, training of new team members, and the continuing education of existing team members. This includes, but will not be limited to:
  • Coordinates periodic case conferences.
  • Participates in the hands-on training of new hires.
  • Identifies pertinent areas of education and assists in the development of corresponding educational programs for existing clinical staff.
  • Keeps informed on the latest updates in state and federal regulations, community standards, and stays abreast of the latest developments in the field of nursing while actively promoting timely dissemination of all developments and changes to the clinical staff. This includes, but will not be limited to:
  • Attendance of offsite regulatory and clinical seminars on an on-going basis.
  • Collaborates with Quality Assurance Department in the dissemination and implementation of new regulatory and clinical guidelines.
  • Actively participates with company committees to achieve desired outcomes including the optimization of OBQI scores, refinement of operating processes, updating existing policies and procedures, enhancing the use of support staff, proper management of supply utilization, supporting marketing efforts, etc.
  • Performs other related duties incidental to the work described herein.
  • Create and approve orders in support of the Field Clinicians.
  • Manage TIF’s, ROC’s and discharges.
  • Review all OASIS information for accuracy.
  • Manage medical supply requisitions.

Qualifications

Completion of an accredited Bachelor's degree in a clinical field.

  • Professional licensure or registration in a clinical field is required.
  • Strong knowledge of Microsoft Office Systems (i.e. Word, Excel, PowerPoint, etc.) and computerized systems in general required
  • Minimum of four years of clinical experience, including supervisory experience in home health care or an equivalent combination of relevant education and/or experience required.
  • Two years in a supervisory position, preferred.
  • Working knowledge of State, Title 22, and Federal Medicare Conditions of Participation regulations.
  • Knowledge and ability to apply community health principles and practices.
  • Ability to exercise independent judgment.
  • Ability to work with individuals.
  • Ability to deal effectively with high levels of stress.
