The CSW care manager will support the North Carolina - Psychiatry Access Line (NC-PAL) and is responsible for clinical expertise for specific complex patient populations with a design to meet specific program related requirements. This role will involve providing behavioral health assessment and consultation, care plan development and facilitation, and referral to appropriate levels of care. The role functions as an integral part of an interdisciplinary team, ensuring excellence in clinical consultation, referral support, and transitions of care to achieve optimal clinical outcomes. The care manager will interface directly with clinicians and/or support services staff within Duke Health and in the community, with the goals of supporting patients, families, and clinicians regarding access to behavioral health resources, addressing systemic barriers to mental health, and supporting patient ability to navigate behavioral health resources. Individuals in this role will also participate in direct patient care (eg., psychosocial assessments, diagnosis, and treatment).

Responsibility


Description



Direct clinical assessment and intervention


  • Coordinate and facilitate timely implementation of behavioral health assessments, care plans, and appropriate interventions for identified patient populations served by NC-PAL to determine patient needs
  • Provide individual and family-based treatment to address barriers and identified concerns by accessing systematically identified data from multiple sources (e.g., medical records, case consultation) to target recipient(s) and provider(s) for outreach education, and intervention
  • Perform targeted interventions to assist patients with connection to health care resources and services
  • Involve the patient and their support systems in the decision-making process; use a patient-centric, collaborative partnership approach to assist the patient and family and identifying barriers by addressing the total individual, inclusive of medical, psychosocial, spiritual, and behavioral needs
  • Implement interventions to ensure patient and family understanding and provide follow-up to support successful implementation of the care plan; apply teaching and learning theories to assist patients and families with the management of behavioral health and other health needs
  • Monitor quality and effectiveness of interventions by setting long- and short-term specific, measurable goals
  • Participate in case reviews and multi-disciplinary health care team assessments
  • Electronically document all activity in the electronic medical record, and in other documentation systems relevant to the program


Clinical consultation and coordination


  • Communicate and coordinate with providers and members of patient care teams as needed to minimize fragmented care and foster appropriate utilization of services and successful transitions of care
  • Facilitate interdisciplinary communication to include specialists, PCP, RN, behavioral health, and other key providers
  • Interface with key providers within the hospital, primary care practices, public health, social service departments, as well as mental health agencies
  • Support the behavioral health consultant in responding to calls from the NC-PAL phone line, including through provision of behavioral health consultative services, referral and care management, and urgent/emergent needs
  • Provide education to clinicians (ex. PCPs, NPs, PAs), clinic staff, community agencies, and/or patients and families regarding behavioral health
  • Participate in meetings of multi-disciplinary health care teams (ex. complex assessments, consultation with observation, etc.)


Outreach


  • Develop and maintain positive relationships with customers internal and external to Duke Health system
  • Engage and inform clinicians and/or support services staff within Duke Health and in the community about NC-PAL
  • Provide onsite, community, and telephonic outreach to patients, providers, and community stakeholders assisting with identification of treatment history, diagnoses and patient care components both internally and externally to ensure that services are sensitive to the needs of patients and take into account ethnic and cultural backgrounds
  • Maintain working knowledge of and liaison with community agencies and resources

Program development


  • Work closely with program administrative core to continually evaluate and improve program implementation activities
  • Attend and participate in staff, committee, department, and other administrative meetings.


Professional development


  • Remain current with health care developments and relevant medical/legal issues that impact on patient care
  • Participate in department, hospital, and medical center continuous quality improvement efforts, regular supervisory conferences, and other professional development efforts identified in conjunction with supervisor(s)


Required Qualifications at this Level

Education/Training

Work requires a Master's degree from an accredited school of social work .

Current licensure as a licensed clinical social worker by the NC Social Work and Certification and Licensure Board.

Experience

Three or more years of social work experience in a health care setting or social service agency with one of three years may be social work internship. Two years recent post master's experience in a clinical setting preferred
