Community Health Worker Job at Preferred Family Healthcare
Please note this position is part of a partnership between Preferred Family Healthcare/Clarity Healthcare and Jordan Valley Community Health Center. If you are hired for this position, you will be considered an employee of Jordan Valley Community Health Center. As a result, your compensation and benefits eligibility will be determined by Jordan Valley Community Health Center.
Your role:
As a Community Health Worker, you will work throughout the service area to build relationships with patient who access the health center or those within the community who are in need of services. The CHW will serve as a liaison or link between patient and resources to facilities access to care.
You will:
- Work directly with patients to identify needs and set client-centered goals.
- Provide individual support to patients working toward self-management of health conditions, primarily in a home or community setting.
- Provide referrals and follow-up on referrals.
- Work directly with a patient's treatment team and participates in care coordination and/or case management.
- Connect patients with resources to meet basic needs and to improve on the patient's social determinants of health.
We think you'd be great for this role if you have:
- Minimum one (1) year experience in the Community Health worker field.
- Bachelor's Degree, preferred.
- Valid Driver's License.
- COVID Vaccine required for all new employees
